摘 要:目的检测血管内皮生长因子(vascular endothelial growth factor,VEGF)及其受体(VEGF receptor,VEGFR)在稽留流产(missed abortion,MA)患者及正常孕妇绒毛组织中的表达,探讨其在稽留流产的发病机制及临床的意义。方法选择深圳市龙岗中心医院2015年2月—9月MA患者96例,试验组1(原因明确)51例、试验组2(原因不明)45例。取同期孕妇57例作为对照组。采用免疫组化方法测定绒毛组织中VEGF、VEGFR-1、VEGFR-2的表达,用平均灰度值(OD值)表示。结果试验组1、试验组2患者绒毛组织中VEGF、VEGFR-1和VEGFR-2的OD值分别为33.08±3.21、35.13±4.05、34.60±4.14和37.69±3.78、36.21±4.13、36.94±4.09,均低于对照组,试验组1 VEGF的OD值低于试验组2,差异有统计学意义(P<0.05)。试验组1、试验组2患者绒毛组织中VEGF、VEGFR-1和VEGFR-2的阳性率分别为29.41%、33.33%、31.37%和37.77%、31.11%、33.33%,均低于对照组,且试验组1 VEGF阳性率低于试验组2,差异有统计学意义(P<0.05)。试验组1 VEGF/VEGFR-1为0.88,低于试验组2,差异有统计学意义(P<0.05)。结论 MA患者绒毛组织中VEGF及其受体VEGFR-1、VEGFR-2的表达呈现低表达,原因明确患者的VEGF表达低于不明确患者,且VEGF/VEGFR-1降低,导致MA发生的机制可能与胚胎血管生成不足有关。Objective To detect vascular endothelial growth factor(VEGF) and its receptor expression levels of villustissues of missed abortion(MA) patients,and explore its pathogenesis mechanism and clinical significance of MA. Methods A total of 96 MA cases were selected in the Longgang Central Hospital of Shenzhen City from February,2015 to September,2015,and they were divided into an Experimental Group 1(known etiology,51 cases) and an Experimental Group 2(unknownetiology,45 cases). Totally 57 pregnant women were selected as a control group during the same period. The expression levelsof VEGF,VEGFR-1,and VEGFR-2 of villus tissues were detected by an immunohistochemistry assay. Results The ODvalues of VEGF,VEGFR-1 and VEGFR-2 of the Experimental Group 1 and Experimental Group 2 were 33.08±3.21,35.13±4.05,34.60±4.14 and 37.69±3.78,36.21±4.13,36.94±4.09 respectively,lower than those of the control group. The VEGF ODvalue of the Experimental Group 1 was lower than that of the Experimental Group 2(P〈0.05). The positive rates of VEGF,VEGFR- 1 and VEGFR- 2 of the Experimental Group 1 and Experimental Group 2 were 29.41%,33.33%,31.37%,and37.77%,31.11%,33.33%,respectively,lower than those of the control group,and the positive rate of VEGF of theExperimental Group 1 was lower than that of the Experimental Group 2(P〈0.05). VEGF/VEGFR-1 of the Experimental Group1 was 0.88,lower than that of the Experimental Group 2(P〈0.05). Conclusion VEGF and its receptor VEGFR- 1 andVEGFR-2 in the villus tissues of MA patients show low expression. The expression level of VEGF in the known etiology MApatients was lower than that in the unknown MA patients,and the VEGF/VEGFR-1 is also lower in the known etiology MApatients,which means that the MA mechanism may be related to inadequate embryonic angiogenesis.
