术后背阔肌复位对乳腺癌改良根治术术后引流量的影响  

Effect of Suturing of the Front Edge of Latissimus Dorsi on Wound Drainage after Modified Radical Mastectomy for Breast Cancer

摘　　要：探讨乳腺癌改良根治术术中缝合背阔肌前缘对术后引流量的影响。选取从2015年7月~2015年12月在笔者所在科室行乳腺癌改良根治术的患者60例,随机分为改进组(30例):在术中改良根治术完成后即时行背阔肌前缘与侧胸壁前锯肌的缝合;对照组仅行标准乳腺癌改良根治术(30例),分析比较两组术后引流量。结果显示改进组患者的中位总引流量(326.03ml)明显少于B组患者的中位总引流量(367.83ml)(P=0.015)。两组的术后引流量在第三天出现明显差异。改进组术后的中位拔管时间(5.66d)明显少于对照组(6.23d)(P=0.009)。缝合背阔肌前缘可以明显减少乳腺癌改良根治术术后的创面渗液,从而缩短患者带管时间,减少住院时间,减轻患者的经济负担和精神负担。To explore the influence of the suture of the front edge of latissimus dorsi on the drainage in the breast cancer patients with modified radical mastectomy.Sixty breast cancer patients selected from Jul 2015 to Dec 2015 undergoing modified radical mastectomy were divided into two groups randomly.Patients in modified group were sutured the front edge of latissimus dorsi and serratus anterior muscle in the lateral thoracic immediately after the modified radical mastectomy（30cases）.Patients in control group only undergoing modified radical mastectomy（30cases）.The volume of drainage was recorded after operation and was analyzed.Result shows that the total median drainage volume in modified group（326.03mL）was significantly lower than control group（367.83mL）（P=0.015）.Volume of drainage in modified group was significantly lower than control group from the third day.The median time to removal the drain tube in modified group（5.66days）was shorter than that in control group（6.23days）（P= 0.009）.The volume of wound drainage can be decreased obviously by suture of the front edge of latissimus dorsi in the breast cancer patients with modified radical mastectomy.The extubation time and hospitalization time of the patients both will be shortened.The economy burden and mental burden of the patients will both be reduced too.
