Foley球囊在预防宫腔粘连电切术后再次粘连的临床研究

摘　　要：目的探讨Foley球囊应用于预防宫腔粘连电切术(transcervical resection of adhesion,TCRA)后再次粘连的临床效果。方法选取我院2014年8月至2015年12月收治的90例中、重度宫腔粘连行TCRA成功分离粘连的患者作为研究对象,随机分为观察组和对照组,各45例。观察组患者于术后放置Foley球囊,对照组患者于术后放置"宫"型金属节育器。比较两组患者3个月后的宫腔再粘连发生情况以及月经恢复情况。结果观察组术后4例患者出现宫腔粘连复发,临床治疗总有效率为91.1%;对照组术后13例患者出现宫腔粘连复发,临床治疗总有效率为71.1%;差异具有统计学意义(P<0.05)。观察组的月经恢复情况明显优于对照组(P<0.05)。结论 Foley球囊应用于预防宫腔粘连电切术后再次粘连的效果明显,能有效促进子宫内膜增长,改善月经情况,预防宫腔再粘连,值得在临床中推广。Objective To explore the clinical effect of Foley balloon to prevent re-adhesion after transcervical resection of adhesion （TCRA）. Methods 90 cases of patients with moderate or severe intrauterine adhesion （IUA） admitted to our hospital from August 2014 to December 2015 were selected and randomly divided into two groups, with 45 cases in each group. All cases received TCRA. After surgery, the observation group was placed with Foley balloon, while the control group was placed with uterine-type metal contraceptive device. 3 months later, the incidence of re-adhesion and the recovery of menstruation were compared between the two groups. Results After surgery, 4 cases had re-adhesion in the observation group, with the total effective rate of 91.1%; 13 cases had re-adhesion in the control group, with the total effective rate of 71.1%; The difference between two groups was statistically significant （P 〈0.05）. The recovery of menstruation of the observation group was significantly better than that of the control group （P〈0.05）. Conclusions Foley balloon has significant efficacy in the prevention of re-adhesion after TCRA, which can effectively promote the increase of endometrium, improve the menstruation situation, prevent re-adhesion, and deserve clinical application and promotion.
