腹部横切口剖宫产术不缝合皮下脂肪层对产后康复和血清细胞因子的影响

摘　　要：目的探讨腹部横切口剖宫产术不缝合皮下脂肪层对产后康复和血清细胞因子的影响。方法选取2013年5月至2015年4月于我院行腹部横切口剖宫产术治疗的足月妊娠的孕产妇300例,随机分为两组各150例。A组不缝合皮下脂肪层直接行皮内连续缝合,B组常规缝合皮下脂肪层再行皮内连续缝合,比较两组的手术时间、术中出血量、术后排气、住院时间,以及术前、术后1 d和术后3 d的血清细胞因子水平。结果 A组患者的手术时间、术中出血量、术后排气时间及住院时间均显著少于B组(P<0.05)。手术后,两组的血清细胞因子(IL-6、IL-8、TNF-α、CRP)水平均较术前高,A组血清细胞因子水平均低于B组(P<0.05)。结论腹部横切口剖宫产术不缝合皮下脂肪层,手术时间短,出血量小,有利于产妇切口愈合,提高生活质量。Objective To investigate the effects of abdominal transverse incision cesarean section without suture of subcutaneous fat layer on postpartum rehabilitation and serum cytokines. Methods 300 cases of pregnant women in term pregnancy in our hospital from May 2013 to April 2015 were selected and randomly divided into two groups, with 150 cases in each group. Group A received intradermal continuous suture directly without suture of subcutaneous fat layer, while group B received intradermal continuous suture after suture of subcutaneous fat layer. The operation time, intraoperative blood loss, postoperative exhaust time, length of hospital stay, and the levels of serum cytokines before surgery, and 1 day and 3 days after surgery were compared between two groups. Results The operation time, postoperative exhaust time and length of hospital stay of group A were significantly shorter than those of group B, and the intraoperative blood loss was significantly less than that of group B （all P 〈0.05）. After surgery, both groups had significantly increased levels of serum cytokines including IL-6, IL-8, TNF-α and CRP （all P 〈0.05）, and the levels of serum cytokines of group A were significantly lower than those of group B （all P 〈0.05）. Conclusions Abdominal transverse incision cesarean section without suture of subcutaneous fat layer has shorter operation time and less blood loss, which is conducive to maternal incision healing and improves the quality of life.
