摘 要:目的评价不同剂量乳化异氟醚延迟预处理对大鼠心肌缺血再灌注损伤的保护作用,并探讨其可能的作用机制。方法健康SD大鼠随机分为假手术组(Sham组)、心肌缺血再灌注损伤组(IR组)、脂肪乳组(IL组)和乳化异氟醚组(EIso组)。各组大鼠尾静脉注射相同容积的0.9%生理盐水、30%脂肪乳和乳化异氟醚2,4,8 mL/kg(液态异氟醚体积比为8%),给药时间30min。给药后24h,结扎左冠状动脉前降支45min,再灌注3h,制作大鼠心肌缺血-再灌注损伤模型。实验结束时,TTC法测定心肌梗死面积、脂质过氧化指标,放射免疫法测血浆内皮素和肿瘤细胞坏死因子(TNF-α)含量变化,免疫组化法测定心肌细胞中TNF-α含量,探讨乳化异氟醚延迟预处理心肌保护的效果和可能的炎性相关机制。结果与模型组相比,乳化异氟醚(4,8mL/kg)组心肌梗死面积明显减少,且呈现剂量依赖性变化,差异有统计学意义(P<0.05);乳化异氟醚(8mL/kg)较2mL/kg组相比,心梗面积下降更为显著,差异有统计学意义(P<0.05)。与假手术组比较,模型组、脂肪乳组和乳化异氟醚组血浆乳酸脱氢酶(LDH)显著升高,差异有统计学意义(P<0.01);相比模型组,4,8mL/kg乳化异氟醚组LDH含量降低,差异有统计学意义(P<0.05)。4,8mL/kg乳化异氟醚组与模型组和脂肪乳组比较内皮素释差异有统计学意义(P<0.05)。与模型组和脂肪乳组比较,4,8mL/kg乳化异氟醚组血浆TNF-a浓度降低,差异有统计学意义(P<0.05)。结论乳化异氟醚延迟预处理能够对大鼠心肌缺血-再灌注损伤具有保护作用,其保护作用呈现剂量依赖性,其机制与抑制心肌脂质过氧化和炎症反应有关。Objective To evaluate the protective effects of different dose of emulsified isoflurane delayed preconditioning on myocardial ischemia reperfusion injury in rats and to investigate possible mechanism.Methods SD rats were randomized into sham group,IR group,IL group and EIso group. Samevolume of 0. 9% saline,30% fat emulsion and emulsified isoflurane 2,4,8mL/kg(volume ratio of liquid isoflurane was 8%) were injected into the caudal vein in each group in 30 minutes.24 hoursafter injection,the left anterior descending coronary artery was ligated for 45 minutes,then reperfusion for 3 hours,and the myocardial ischemiareperfusion injury model was established.At the end of the experiment,the myocardial infarction size and lipid peroxidation index were measured by TTC method.The levels of plasma endothelin(ET) and tumor necrosis factor(TNF-α) were measured by radioimmunoassay.TNF-αin myocardia cells was determined by immunohistochemistry.And the protective effects of emulsified isoflurane delayed preconditioning and possible inflammatory mechanisms were investigated. Results Compared with the model group,the infarction size was significantly decreased in emulsified isoflurane(4,8 mL/kg) group(P〈0. 05),and the decrease was dose-dependent.Compared withemulsified isoflurane(2mL/kg) group,the infarction size in the emulsified isoflurane(8mL/kg)groupdecreased more significantly(P〈0. 05).Compared with the sham group,the plasm level of lactate dehydrogenase(LDH) in the model,fat emulsion and the emulsified isoflurane group increased significantly(P〈0. 01).Compared with the model group,the level of LDH was significantly lower in emulsified isoflurane group(4,8mL/kg).And the endothelin level in emulsified isoflurane group(4,8mL /kg) was different from that in the model and fat emulsion group,with statistical significance(P〈0. 05).The plasma concentration of TNF-α in emulsified isoflurane group(4,8mL/kg) significantly decreased(P〈0. 05).Conclusion Delayed pre
