糖尿病肾病患者心脏瓣膜钙化危险因素分析

摘　　要：目的观察糖尿病肾病患者心脏钙化与临床相关指标,阐明糖尿病肾病患者心脏瓣膜钙化的相关危险因素。方法选择我院2014年1月至2015年8月住院的糖尿病肾病患者80例,根据心脏瓣膜钙化有无情况分为钙化组、非钙化组,比较两组有关临床参数的变化,分析钙化相关危险因素。结果瓣膜钙化组和非钙化组比较,性别、吸烟率、糖化血红蛋白、血钙、钙磷乘积、甘油三酯、胆固醇、高密度脂蛋白、尿素氮、血肌酐差异无统计学意义(P>0.05),两组的年龄、平均动脉压、血磷、iPTH、血白蛋白比较,差异有统计学意义(P<0.05),多因素Logistic回归分析显示年龄、平均动脉压、血磷、iPTH是糖尿病肾病患者心脏瓣膜钙化主要危险因素,血浆白蛋白是保护性因素。结论年龄、平均动脉压、血磷、iPTH、白蛋白水平是影响糖尿病肾病患者心脏瓣膜钙化的因素。Objective Cardiac calcification in patients with diabetic nephropathy and the associated clinical indicators were observed,which can clarify the related risk factors of cardiac valvular calcification for diabetic nephropathy patients.Methods 80 cases of diabetic nephropathy patients selected from January 2014 to August 2015 in the sixth people＇shospital of Chengdu were divided into calcification and non-calcification group according to the situation of cardiac valvular calcification.The changes of clinical parameters of two groups were compared and the calcification related risk factors were analyzed.Results Compared ofvalvular calcification and non-calcified group,the differences of gender,smoking rate,glycated hemoglobin,serum calcium,calcium-phosphorus product,triglyceride,cholesterol,high-density lipoprotein,blood urea nitrogen and serum creatinine were no statistically significant（ P〉0. 05）. Comparing the age,mean arterial pressure,serum phosphate,iPTH,blood albumin of two groups,the difference was statistically significant（ P〈0. 05）. The multi-factor Logistic regression analysis showed that the age,mean arterial pressure,serum phosphate and iPTH were major risk factors for diabetic nephropathy patients with cardiac valvular calcification,plasma albumin was protective factor.Conclusion Age,mean arterial pressure,Serum phosphate,iPTH and albumin levels are the influencing factors of cardiacvalvular calcification for diabetic nephropathy patients.
