益肾和血调周法对多囊卵巢综合征肥胖者和非肥胖者胰岛素抵抗的影响

摘　　要：目的:观察益肾和血调周法内外合治对多囊卵巢综合征(PCOS)肥胖者和非肥胖者胰岛素抵抗的影响。方法:将60例PCOS患者分为肥胖组和非肥胖组,运用益肾和血调周法,内服中药辅以外用暖宫活血方熏蒸治疗,分别于治疗前、治疗后第1个、第2个疗程结束后,检测空腹血糖(FPG)、餐后2h血糖(P2h PG)、胰岛素(FINS)(空腹、60min、120min)及胰岛素抵抗指数(HOMA-IR=FINS×FPG/22.5)。结果:肥胖组在治疗后P2h PG、FINS(空腹、60min、120min)、HOMA-IR较治疗前均明显下降(P<0.05),但FPG治疗前后差异无统计学意义;非肥胖组治疗前后FINS(空腹、60min、120min)、HOMA-IR较治疗前明显下降(P<0.05),P2h PG在第2个疗程结束后较治疗前有明显下降,但FPG治疗前后差异无统计学意义。结论:益肾和血调周法可有效改善PCOS肥胖者与非肥胖者的胰岛素抵抗,进一步证实益肾和血调周法内外合治在治疗PCOS中的有效性和优势。Objective： To investigate the effect of combined external and internal treatment with Yishen Hexue Tiaozhou method on insulin resistance in obese and non- obese patients with polycystic ovary syndrome（ PCOS）.Methods： A total of 60 PCOS patients were divided into obesity group and non- obesity group and the YishenH exue Tiaozhou method was applied. The patients were treated with oral administration of traditional Chinese medicine and external application of Nuangong Huoxue prescription for fumigation and steaming therapy. Fasting plasma glucose（ FPG）,2- hour postprandial blood glucose（ P2 h PG）,fasting insulin（ FINS）,60- and 120- minute insulin,and insulin resistance index [homeostasis model assessment- estimated insulin resistance（ HOMA- IR） = FINS ×FPG /22. 5] were measured before treatment and at the ends of the first and second courses of treatment. Results：The obesity group had significant reductions in P2 h PG,FINS,60- and 120- minute insulin,and HOMA- IR after treatment（ P〈0. 05）,but there was no significant change in FPG after treatment（ P〈0. 05）. The non- obesity group had significant reductions in FINS,60- and 120- minute insulin,and HOMA- IR after treatment（ P〈0. 05）,and there was a significant reduction in P2 h PG at the end of the second course of treatment; there was no significant change in FPG after treatment. Conclusion： Yishen Hexue Tiaozhou method can effectively improve insulin resistance in obese and non- obese PCOS patients,which further verifies the effect and advantages of combined external and internal treatment with Yishen Hexue Tiaozhou method in the treatment of PCOS.
