低管电压在冠状动脉CT低剂量成像中的应用  被引量:1

摘  要:目的探讨在正常体质量指数患者中,使用低管电压联合管电流自动调节技术在volumek模式640层CT冠状动脉血管成像中实现低辐射剂量的可行性。方法疑诊冠状动脉疾病行640层CT冠状动脉血管成像检查者60例,心率为51—67次/min,BMI为18.5—25kg/m^2,体重〈70kg。按数字表法随机分为A、B、C三组,A组采用kV自动调节,mA自动调节(60-390mA),B组采用100kv,mA自动调节(60-220mA)。C组采用80kV,mA自动调节(60-220mA)。采用volume的prosp CTA模式,采集一个心动周期R-R间期的70%-80%时相。记录各组的CT值、剂量长度乘积(DLP)、噪声(SD),并对有效剂量(ED)、信噪比(SNR)、对比噪声比(CNR)等进行统计学检验。结果A组ED0.97mSv,B组ED0.47mSv,C组ED0.40mSv。图像噪声A组与C组比较,差异有统计学意义(P〈0.05),左、右冠脉的SNR及时对CNR三组比较.差异无统计学意义(P〉0.015);主观评分优良冠状动脉血管比例三组之间的差异无统计学意义。结论在不影响诊断的前提下,低管电压80kV联合管电流自动调节技术在正常体质量指数患者中降低辐射剂量是可行的。Objective To investigate the feasibility of low doses of radiation in patients with normal body mass index ( bmi ) , using lower tube voltage joint tube current automatic adjustment technology in volume model 640 C T imaging in coronary arteries. Methods 60 suspected patients with coronary artery disease in our hospital underwent 640 CT coronary angiography, the heart rate of 51-67 times per minute, BMI were 18.5-25kg/ m^2, weight〈70kg. Press numeric table method were randomly divided into A, B, C three groups, group A with kV automatic adjustment, mA automatic adjustment ( 60-390mA ) , group B with 100kV, mA automatic adjustment ( 60-220 mA ) , group C with 80 kV, mA automatic adjustment ( 60- 220mA ) . Using volume prosp CTA mode, a cardiac cycle between R-R period of 70-80% was gathered. Records of each product of CT values, dose length ( DLP ) , noise ( SD ) , and the effective dose ( ED ) , signal-to-noise ratio ( SNR ) , contrast to noise ratio ( CNR ) , and other statistical inspection. Results Group A ED was 0.97 mSv and group BED was 0.47 mSv, group C ED was 0.40 mSv. The comparison of image noise A, C ( P〈0.05 ) was statistically significant, left, fight coronary artery of SNR and three groups of CNR was no statistical differences (P〉0.015) .Good subjective ratings of coronary arteries no statistically significant difference between three groups. Conclusion Without affecting the diagnosis, it is feasible to reduce the radiation dose in patients with normal body mass index under the premise of lower tube voltage 80 kv joint tube current automatic adjustment technology.
