人工真皮结合自体刃厚皮片修复小面积Ⅳ°烧伤疗效观察

摘　　要：目的探讨人工真皮结合自体刃厚皮片修复肢体远端小面积Ⅳ°烧伤创面的效果。方法2013年1月至2016年10月肢体远端形成明确Ⅳ。创面患者7例，均采用人工真皮植入后延迟自体刃厚皮片移植方法修复，观察植皮成活率，受区功能活动及供区瘢痕增生情况。结果皮片均Ⅰ期成活，成活率96％-100％，受区创面柔软度好，无明显挛缩，对周围关节活动度影响小。供皮区预后无明显瘢痕增生，随诊1-6个月外观满意。结论应用人工真皮结合自体刃厚皮片修复肢体远端小面积Ⅳ°烧伤创面，供区仅需提供刃厚皮片，损伤极小，新创面愈合迅速，预后无瘢痕增生，优点显著，疗效确切，值得肯定并推广。Objective To discuss effect of artificial dermis combined with self split-thickness skin grafting to repair distal limb small area IV degree burn wounds. Methods From January 2013 to October 2016, our department received and cured 7 cases of distal limb IV degree burn wounds, all the cases conducted artificial dermis combined postpone self split-thickness skin grafting, skin graft survival rate, scar hyperplasia and functional activity of recipient site and donor site were observed. Results All the cases skin graft were alive in stage I. skin graft survival rate were 96%-100%. Recipient site were soft, no obvious contracture occurred, and few influences of around the joints activity. 1 to 6 months Later, donor site had no scar hyperplasia, appearance were good. Conclusion Our method of donor site split-thickness skin to repair distal limb small area IV degree burn wounds were rapid healing, and no scar hyperplasia. The method has significant advantages and exact curative effect, it＇ s worth promoting.
