经食道超声心动图监测在围术期的应用

摘　　要：介绍了经食道超声心动图(transesophageal echocardiography,TEE)监测在围术期中的应用,阐述了在非心脏手术中的一些新功能。分析了TEE监测技术在先心病、冠心病、瓣膜病、主动脉疾病等心脏手术和非心脏手术中的优缺点。解决了以往单凭经验判断患者心功能及容量负荷情况从而导致盲目使用血管活性药或补液的问题。指出该监测技术可帮助麻醉医生对很多患有心脏疾病的患者进行诊断评估,也可在术中协助指导外科医生即刻评估手术效果,以及在许多非心脏手术中能够实时监测患者的心功能及血流动力学,已成为麻醉医生在围术期监测的"第三只眼睛"。The application of transesophageal echocardiography（TEE） in perioperative monitoring was introduced, and its involvement in non-cardiac surgery was described. The advantages and disadvantages of TEE were analyzed when applied to cardiac and non-cardiac operations for congenital heart disease, coronary disease, vascular heart disease, aorta disease and etc. The abuse of vasoactive drug or fluid is avoided due to subjective evaluation on cardiac function and volume load. TEE facilitates the anesthetist in diagnosis and evaluation of the patient with heart diseases, and the surgeon in immediate assessment of the operation, which can be used for real-time monitoring of cardiac function and hemodynamics.
