摘 要:目的经阴道超声监测卵泡及卵巢动脉的周期性变化,对不孕症妇女做出评估,指导临床对症治疗。方法对我院2015年8月~2016年8月间前来就诊的原发性不孕症妇女90例202个周期和育龄期正常女性卵泡监测90例作对比,观察两组女性卵泡及卵巢动脉周期性变化的不同。结果 90例原发性不孕症妇女中,正常排卵51例,卵泡平均直径(19.75±1.98)mm,17例未见明显卵泡,8例为黄素化未破裂卵泡综合征,6例为小卵泡排卵,8例为多囊卵巢综合征;对照组平均卵泡直径(20.93±2.04)mm,与观察组中正常排卵者平均卵泡直径相比,差异无统计学意义(P>0.05)。不孕症妇女中正常排卵者及对照组中妇女卵巢动脉流速和血流阻力指数RI呈周期性变化,观察组中黄素化卵泡未破裂综合征、未见明显卵泡和多囊卵巢综合征患者的卵巢动脉卵泡期、黄体期血流的平均血流速度均明显低于对照组及观察组中可正常排卵者(P<0.05),而卵巢动脉卵泡期、黄体期血流的RI值变化无统计学意义(P>0.05),小卵泡排卵者的卵巢动脉平均血流速度及RI周期性变化与对照组比较,差异无统计学意义(P>0.05)。结论经阴道超声监测原发性不孕症妇女卵泡及卵巢动脉周期性变化,可以为临床用药提供合适时机,更好的治疗不孕症。Objective By monitoring the changes of follicular and ovarian arteries by transvaginal uhrasonography,to evaluate the infertility of women,and to guid the clinical treatment. Methods 90 cases (202 cycles) of primary infertility women cured in our hospital from Auguest 2015 to Auguest 2016 were selected to compare with 90 cases of follicular of normal fertile women.The changes of follicular and ovarian arteries in two groups were observed. Results Among the 90 cases of primary infertility, there were 51 cases of normal ovulation(The average follicle diameter was (19.75 ±1.98)mm,17 cases of no obvious follicle,8 cases of luteinized unruptured follicle syndrome,6 cases of small follicle ovulation, and 8 cases of polyeystic ovary syndrome.The average follicular diameter of control group was (20.93 ±2.04) mm,compared with the average follicular diameter of normal ovulation in observation group,the difference was not statistically significant(P 〉 0.05).Ovarian arterial flow velocity and flow resistance index RI of women with normal ovulation and infertile women in control group showed periodic changes.The lmeinized unruptured follicle syndrome,the average blood flow velocity not obvious in patients with polycystic ovary syndrome and follicular ovarian artery blood flow in the follicular phase and luteal phase of observation group were significantly lower than those of control group and observation group in normal ovulation women(P 〈 0.05).There was no significant change in RI value of ovarian artery during follicular phase and luteal phase(P 〉 0.05).There was no significant difference in mean arterial blood flow velocity and RI periodicity between the ovarian arteries and the control group(P 〉 0.05). Conclusion Monitoring the changes of follicular and ovarian arteries by transvaginal ultrasonography of primary infertility women can provide the right time for clinical medication for better treatment of infertility.
