摘 要:目的探讨血清γ-谷氨酰转移酶(γ-GT)水平与冠状动脉钙化(CAC)的关系,为临床评估CAC提供新依据。方法选择扬州市江都人民医院心内科182例行冠状动脉CT造影检查的住院患者,收集临床资料,检测相关生化指标,计算CAC积分;根据γ-GT水平四分位数分为4组(第1四分位组:γ-GT≤19 U/L、第2四分位组:19 U/L<γ-GT≤25 U/L、第3四分位组:25 U/L<γ-GT≤37 U/L、第4四分位组:37 U/L<γ-GT≤62 U/L);并依据CAC积分进一步分为CAC积分≥1(钙化组)和CAC积分=0(非钙化组)。结果随γ-GT水平升高,4组间的吸烟率、男性比例、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、尿酸(UA)、高敏C反应蛋白(hs-CRP)、空腹血糖(FBG)和糖化血红蛋白(Hb Alc)水平依次升高,而高密度脂蛋白胆固醇(HDL-C)逐渐降低,差异有统计学意义(均为P<0.05);且各组间两两比较,差异有统计学意义(均为P<0.05)。γ-GT与UA、TC、hs-CRP、Hb Alc呈正相关,与HDL-C呈负相关。年龄、性别、高血压、吸烟、FBG、γ-GT、UA均与CAC有显著相关性;且γ-GT、年龄、吸烟、UA是CAC的独立预测因子(OR值分别为1.13、1.21、3.98、1.62,均为P<0.05)。结论血清γ-GT水平与CAC有相关性。Objective To investigate the relationship between serum γ-glutamyhransferase(γ-GT) level and coronary artery calcification (CAC), and to provide new evidence for the clinical evaluation of CAC. Methods A total of 182 inpatients underwent coronary computed tomography angiography (CTA) were enrolled from Department of Cardiology, Jiangdu People's Hospital, Yangzhou. The clinical data were collected, the biochemical parameters were measured. The patients were classified into 4 quartile groups according to their serum γ-GT level( The first quartile :γ-GT ~〈 19 U/L, the second quartile : 19 U/L 〈 γ-GT ≤25 U/L,the third quartile:25 U/L≤-GT~〈37 U/L,the fourth quartile group:37 U/L 〈 γ-GT≤62 U/L, and further divided into CAC score≥ 1 (calcification group)and CAC score = 0 group. Results With the increase of γ-GT level, smoking rate, male ratio, total cholesterol ( TC ) , low density lipoprotein cholesterol (LDL-C) ,uric acid (UA), high-sensitivity C reactive protein (hs-CRP)and fasting blood glucose (FBG), glycosylated hemoglobin(HbAlc) of the four groups were increased, while high-density lipoprotein( HDL-C ) decreased( all P 〈0. 05 ) ,and the difference between the two groups was statistically significant(P 〈0. 05 ) ; serum y-GT levels were positively correlated with smoking rate, male ratio, UA, TC, hs-CRP, HbAle and negatively correlated with HDL-C ; while no correlations with age, diabetes mellitus, hypertension, FBG, LDL- C, Cr( all P 〈 0. 05 ). Age, gender,hypertension, smoking, FBG, γ-GT, UA levels were significantly cmTelated with presence of CAC;and γ-GT,age, smoking and UA levels appeared as independent factors predictive of presence of CAC( OR values were 1.13,1.21,3.98,1.62,all P 〈0.05). Conclusions There is a significant correlation between γ-GT and CAC.
