体外循环心脏瓣膜术后患者应对方式与疾病不确定感的相关性分析

摘　　要：目的分析体外循环心脏瓣膜术后患者应对方式与疾病不确定感及其相关性。方法应用医学应对方式问卷、疾病不确定感问卷对208例体外循环心脏瓣膜术后患者进行调查。结果体外循环心脏瓣膜术后患者面对维度得分低于常模(P<0.01),回避和屈服维度得分高于常模(P<0.01或P<0.05)。患者疾病不确定感得分为(90.33±10.32)分,体外循环心脏瓣膜术后患者面对应对方式与疾病不确定感水平呈负相关,而回避、屈服应对方式与疾病不确定感水平呈正相关(P<0.01或P<0.05)。结论体外循环心脏瓣膜术后患者多采取回避和屈服的应对方式,患者疾病不确定感处于中等水平,护理管理者应鼓励患者寻求疾病知识与配合治疗,进而降低患者疾病不确定感水平。Objective To analysis the correlation between copping style and uncertainty in patients with heart valve surgery under extraeorporeal circula- tion. Methods A total of 208 patients with heart valve surgery under extracorporeal circulation were investigated with Medical Coping Modes Questionnaire （ MC- MQ） and Mishel Uncertainty in Illness Scale（MUIS）. Results The score of confrontation in cardiac valvular postoperative patients under cardiopulmonary by- pass was significantly lower than clinical routine（ P 〈 0.01 ）, the score of avoidance and the yield dimension were higher than the norm （ P 〈 0.01 or P 〈 0.05 ）. The score of uncertainty in illness was （ 90.33 ＋ 10.32 ）. The copping style was negatively correlated with uncertainty in illness. The coping modes of avoidance and yield were positively correlated with uncertainty （ P 〈 0.01 or P 〈 0.05 ）. Conclusion The coping modes which cardiac valvular postoperative patients with cardiopulmonary bypass were avoidance and yield. The uncertainty in illness is at the middle level. Nursing managers should guide patients to seek illness related knowledge and treated by combining, thus to reduce the level of uncertainty in illness.
