摘  要:目的检测类风湿关节炎(RA)患者血清中趋化因子CCL19的表达水平,并探讨其临床意义。方法收集2014年3月—10月中国医科大学附属盛京医院风湿免疫科住院的40例RA患者,18例骨关节炎(OA)患者和18名健康对照者。采用酶联免疫吸附试验(ELISA)法测定40例RA患者、18例OA患者和18名健康对照者血清中CCL19水平,比较组间差异,并分析血清CCL19与RA各临床及实验室指标的相关性。结果 RA组、OA组及对照组各组间血清CCL19水平差异有统计学意义(P<0.01)。RA患者血清CCL19水平与疾病活动度评分(DAS28)、C反应蛋白(CRP)、红细胞沉降率(ESR)、类风湿因子(RF)、血小板(PLT)、免疫球蛋白Ig M呈正相关(P<0.05),与血红蛋白(Hb)呈负相关(P<0.05)。结论血清CCL19可能参与了RA的发病过程,有助于RA的鉴别诊断。Objective By testing serum levels of C-C motif chemokine 19 (CCL19) in patients with rheumatoid arthritis (RA), the study investigates the role and clinical significance of serum levels of CCL19 in the pathogenesis of RA. Methods 40 patients with RA were enrolled in the department of rheumatology and immunology3 Shengjing Hospital, China Medical University from March to October in 2014, and a total of 18 patients with osteoarthritis (OA) and 18 healthy controls were enrolled. The serum levels of CCL19 in patients with RA and patients with osteoarthritis (OA) and healthy controls were determined by enzyme-linked immunosorbent assay (ELISA). Results CCL19 levels had statistical differences(P〈0.01) among patients in control group, OA and RA. The levels of CCL19 in RA patients were positively related to disease activity score (DAS28), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), rheumatoid factor (RF), platelet (PLT) and immunoglobulin IgM (P 〈0.05), but negatively correlated with hemoglobin (Hb) (P〈0.05). Conclusion CCL19 is involved in the pathogenesis of RA, which helps to make differential diagnosis of RA.
