综合疗法治疗下肢外伤截肢后幻肢痛的临床研究

摘　　要：目的评价牛痘疫苗致炎兔皮提取物注射液联合高乌甲素贴片、神经损伤治疗仪、冲击波治疗仪、心理疗法及镜像运动疗法治疗下肢外伤截肢后幻肢痛的临床疗效。方法 52例下肢外伤后截肢术后伴发幻肢痛患者,分为对照组和观察组,二组均接受高乌甲素贴片双耳后贴敷治疗(2贴/72h)、患肢神经治疗仪局部治疗(2次/d)、冲击波疼痛治疗(1次/d)、心理辅助治疗及镜像运动疗法治疗(1次/d),此外观察组应用牛痘疫苗致炎兔皮提取物注射液(3.6U,2次/d肌注),对照组予以维生素B12注射液(0.1mg,1次/d肌注),疗程均为2周。治疗后以简化McGill疼痛问卷(SF-MPQ)进行镇痛疗效评定,以90项症状自评量表(SCL-90)进行心理状态评定。结果二组治疗后SF-MPQ中的疼痛指数(PRI)、疼痛模糊评分(VAS)和疼痛程度(PPI)三项积分及SCL-90综合心理评分均优于治疗前(P<0.05),而观察组各项积分好于对照组(P<0.05)。结论牛痘疫苗致炎兔皮提取物注射液联合高乌甲素贴片、神经损伤治疗仪、冲击波疼痛治疗、心理疗法及镜像运动疗法治疗外伤后下肢截肢幻肢痛疗效确切。Objective To observe the effect of extracts from rabbit skin inflamed by Vaccinia virus for injection,lappaconitine adhesive patch,nerve injury therapeutic instrument,shock wave therapy,psychotherapy and mirror therapy in phantom limb pain of lower limbs after traumatic amputation.Methods Fifty-two patients with phantom limb pain after traumatic amputation were equally divided into treatment group and control group.All patients received Lappaconitine adhesive patch（2 patches/72h）combined with nerve injury therapeutic instrument（2/d）,shock wave therapy（1/d）,psychotherapy（1/d）and mirror therapy（1/d）.Treatment group received injection of Extracts from Rabbit Skin Inflamed by Vaccinia Virus for Injection（3.6U,2/d）and control group was treated with Vitamin B12 injection（0.1mg,1/d）for 2 weeks.The shot-form of McGill questionnaire（SF-MPQ）and symptom checklist 90 profiles（SCL-90）were used to measure painess and psychological rating.Results The scores of SCL-90 and PRI,VAS and PPI in SF-MPQ in both groups were much lower after treatment（P〈0.05）,but treatment group gained better scores（P〈0.05）.Conclusion It is effective in the treatment of phantom limb pain in lower limbs after traumatic amputation by extracts from rabbit skin inflamed by Vaccinia Virus for injection combined with Lappaconitine adhesive patch,nerve injury therapeutic instrument,shock wave therapy,psychotherapy and mirror therapy.
