摘 要:目的评估右美托咪定对大鼠急性高氧性肺损伤(HALI)的影响。方法将32只Wistar雄性大鼠随机数字表法分为4组(n=8):健康对照组(C组);急性高氧肺损伤模型组(HALI组);右美托咪定治疗组(D+HALI组);生理盐水治疗组(Sham组)。C组不做任何处理,采取95%浓度氧气自主呼吸48 h制作HALI模型组,D+HALI组在完成HALI模型后,即刻股静脉持续输注右美托咪定2μg/(kg·h),Sham组持续输注生理盐水0.5m L/(kg·h)。治疗8 h后,使用10%的水合氯醛麻醉大鼠,经胸主动脉采集动脉血做血气分析;取右肺上叶测试湿干比(W/D);取右肺中叶和下叶HE染色,光镜下观察肺组织病理学变化并进行肺损伤评分;取左肺组织匀浆后取上清液进行ELISA检测髓过氧化物酶(MPO)、巨噬细胞炎症蛋白2(MIP-2)、白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)浓度。结果 (1)与C组比较,HALI组动脉血气PH值、PO2值降低,PCO2升高,肺组织损伤程度与病理评分、W/D及肺组织中MPO、MIP-2、IL-1β、TNF-α含量显著性增加,差异有统计学意义(P<0.05);(2)与HALI组比较,D+HALI组PH值、PO2值增加,PCO2值下降,肺组织损伤程度与病理评分、W/D及肺组织中MPO、MIP-2、IL-1β、TNF-α含量显著性下降,差异有统计学意义(P<0.05);(3)与HALI组比较,Sham组的各项指标差异无统计学意义(P>0.05)。结论右美托咪定可能减轻大鼠急性高氧性肺损伤、抑制炎症物质浸润。Objective To evaluate the protective effect of dexmedetomidine on hyperoxia-induced of acute lung injury (HALI) in rats. Methods The 32 male Wistar rats were randomly divided into 4 groups(n=8):heahhy control group(group C);hyperoxia-acute lung injury model group (group HALI);Dexmedetomidine treatment group (group D+HALI);normal saline treatment group (group Sham).Group C wasn't given any treatment.Take 95% concentrations of oxygen to breathe 48 hours to make the group HALL Group D+HALI femoral venous continuous infusion of dexmedetomidine [2 μg/(kg·h)] after the completion of HALI model. Group sham continuous infusion of saline [0.5 ml/(kg·h)].After 8 hours of treatment, the rats were anesthetized with 10% of the chlorine hydrate,and blood was collected by the thoracic aorta, and then arterial blood was used for blood gas analysis;Wet to dry ratio (W/D) of the upper lobe of the right lung was taken;HE staining was carried out in the middle and lower lobes of the right lung, and the pathological changes of lung tissue were observed under light microscope;The left lung tissue was removed and the supematant was removed after centrifugation,Using ELISA method for the detection of myeloperoxidase (MPO), macrophage inflammatory protein 2 (MIP-2), interleukin(1 beta (IL-1β),tumor necrosis factor alpha (TNF-α) concentration. Results (1) Compared with the group C, group HALI of arterial blood gas results suggested that the pH value, POE value decreased,PC02 value increased.The degree of lung tissue injury and the pathological score,MPO,MIP-2,IL-1β,TNF-α,W/D content increased significantly (P 〈 0.05);(2)Compared with HALI group, group D+HALI blood gas analysis showed that:pH value,PO2 value increased, PCO2 value decreased.
