针灸联合药熨治疗腰椎间盘突出症的临床观察

摘　　要：目的为针灸联合中药热熨治疗腰椎间盘突出症患者的疗效提供临床依据。方法辽宁中医药大学附属医院骨一科在2011年3月~2014年9月间对符合腰椎间盘突出症诊断,且中医辨证为寒凝血瘀的60例住院患者,单盲随机分为两组:对照组30例,治疗组30例。治疗方式:对照组为基础治疗(包括急性期腰部制动、后期腰背肌功能训练、中药熨法治疗、激素及脱水药物静点),治疗组在基础治疗上加用针灸疗法。两组进行2周临床治疗后,以VAS视觉模拟评分法、Oswestry腰椎疾患功能障碍指数调查问卷(ODI)评分改善率为主要评价指标,进行疗效比较。结果对照组及治疗组治疗后的VAS评分[(3.67±1.14),(2.44±0.97)分]、ODI评分[(36.87±11.16),(28.85±10.17)分]分别与治疗前的VAS评分[(6.21±1.34),(6.23±1.26)分]、ODI评分[(52.17±11.89),(51.15±12.13)分]比较,差异有统计学意义(P<0.05)。治疗后,治疗组VAS评分及ODI评分均显著低于对照组,差异有统计学意义(P<0.05)。结论针灸联合中药热熨治疗腰椎间盘突出症在缓解患者疼痛及改善生活质量上疗效确切,相比单一的外治方法,能加速肢体功能的恢复。Objective To provide clinical evidence for the treatment of acupuncture therapy combined with hot compress therapy of traditional Chinese medicine on the patients with lumbar disc herniation. Methods 60 patients with both lumbar disc herniation and blood stasis due to cold in TCM syndrome for ward treatment in the 1st section of Orthopaedics Department, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from March 2011 to September 2014 were selected as research objects and divided into two groups according to the single-blind and random-control method,each group had 30 cases. Treatments： the control group was treated with basic treatments, including lumbus immobilization during the acute stage, functional exercise of back muscles in the later stage,hot compress therapy of traditional Chinese medicine and intravenous drip of hormone and dehydration drugs. The treatment group was treated with basic treatments combined with acupuncture therapy. The improvement rates of Visual analogue scale （VAS） and Oswestry disability index （ODI） before and after 2 weeks treatment were compared between the two groups, and clinical curative effect was compared between two groups. Results After treatment,VAS [（6.21±1.34）,（6.23±1.26） points],[（52.17±11.89）,（51.15±12.13）points]and ODI [（3.67±1.14）,（2.44±0.97）points],[（36.87±11.16）,（28.85±10.17）points] score in treatment group and control group were compared respectively,with statistical differences（P 〈 0.05）.After treatment, VAS and ODI score in treatment group are lower than that of control group,there were statistically significant differences（P 〈 0.05）. Conclusion Acupuncture therapy combined with hot medicine compress therapy of traditional Chinese medicine in the treatment of lumbar disc herniation has exact therapeutic effect in the easement of pain and improvement of quality of life.It can accelerates the recovery of limb function by compared with single external treatment.
