摘 要:目的探讨F波在糖尿病周围神经病变(DPN)中的影响因素。方法收集DPN患者159例,根据HbA_1c水平分为HbA_1c≤7%组,HbA_1 c>7%组,根据是否合并高血压病分为高血压病组和无高血压病组。检测患者的FPG、HbA_1 c,采用肌电图诱发电位仪检测胫神经的F波最短潜伏期(Fmin),F波最长潜伏期(Fmax),F波平均潜伏期(Fmean),F波出现率(Fpresent),并进行比较。结果159例DPN患者的FPG、HbA_1 c与Fmin、Fmax、Fmean均呈正相关(P<0.01)。与无高血压病组比较,高血压病组Fmin延长[(44.52±7.17)vs(46.90±6.49)ms,P<0.01]。与HbA_1 c≤7%组比较,HbA_1 c>7%组Fmin延长[(44.10±5.12)vs(46.82±7.15)ms,P<0.05],Fmax延长[(50.68土5.46)vs(55.09±6.19)ms,P<0.01],Fmean延长[(46.92±5.33)vs(50.96±5.58)ms,P<0.01]。结论高血糖状态及高血压合并症均可加剧DPN患者F波的异常改变。Objective To investigate the influencing factors for F wave in diabetic peripheral neuropathy(DPN).Methods A total of 159 patients with DPN were recruited and divided into two groups according the level of hemoglobin HbA1 c:HbA1 c≤7%and HbA1 c〉5%group.In addition,all the subjects were also divided into two groups according to whether complicated with hypertension:hypertension group and no hypertension group.The fasting plasma glucose(FPG) and HbA1 c were tested in all the patients.The parameters of tibial nerve F wave such as minimum latency(Fmin),the maximum latency(Fmax),the mean latency(Fmean),and the probability of F wave present(Fpresent) were recorded by EMG evoked potential instrument.Results In all the 159 patients with DPN,the FPG and HbA1C were significantly positively correlated with Fmin,Fmax and Fmean(P〈0.01).The Fmin was significantly prolonged in hypertension group than in no hypertension group[(44.52 ± 7.17) vs(46.90 ±6.49) ms,P〈0.01].Fmin[(44.10±5.12) vs(46.82 ± 7.15)ms,P〈0.05],Fmax[(50.68±5.46) vs(55.09±6.19)ms,P〈0.01],and Fmean[(46.92 ± 5.33) vs(50.96 ± 5.58) ms,P〈0.01]were also significantly prolonged in HbA1c7%group than in HbA1 ≤7%group.Conclusion Hyperglycemia and hypertension complication may aggravate the abnormal changes of F wave in patients with DPN.
