摘 要:目的探讨血小板参数对感染性休克患者预后的预测作用。方法回顾分析80例感染性休克患者的临床资料。应用多因素Logistic回归模型分析死亡优势比(OR),并绘制血小板相关参数,如血小板计数(PLT)、血小板分布宽度(PDW)、血小板压积(PCT)及血小板平均体积(MPV)的受试者工作特征曲线(ROC曲线)。结果入院当天死亡组APACHEⅡ评分较存活组高(P<0.01)。两组患者PLT、PCT和PDW差异无统计学意义(P>0.05)。死亡组MPV整个病程呈上升趋势,明显高于存活组(P=0.014)。MPV、APACHEⅡ评分+MPV预测病死率的敏感度分别为83.31%、86.70%,特异度分别为64.50%、72.30%。结论血小板参数MPV结合APACHEⅡ评分可以预测感染性休克患者的预后。Objective To investigate the predictive effect on prognosis of platelet parameters in patients with septic shock. Methods The clinical data of 80 patients with septic shock were retrospective analyzed. The multivariate Logistic analysis was used to analyze the mortality ratio(OR), and the receiver operating characteristic eurve(ROC cover) was drawn for PLT, platelet distribution width (PDW), plateletcrit (PCT) and meam platelet volume(MPV). Results The APACHE Ⅱ score in death group was higher than that in survivor group on the day of admission(P〈0.01). There were no statistical differences between death group and survival group in the PLT, PCT and PDW. The MPV in the death group increased gradually in the whole course of the disease, and was significantly higher than that in the survival group(P= 0.014). The sensitivities of MPV and APACHE Ⅱ seore+MPV in predicting the mortality were 83.31% and 86.70%, respectively, and the specificities was 64.50% and 72.30%, respectively. Conclusion The platelet parameter MPV combined with APACHE Ⅱ score can predict the prognosis of patients with septic shock.
