摘 要:目的评价肿瘤特异性生长因子(Tumor specific growth factor,TSGF)和卵巢恶性肿瘤风险模型(Risk ovarian malignancy algorithm,ROMA)指数对卵巢癌风险性的评估价值并分析两项指标与卵巢癌临床病理分期的相关性,为临床诊断治疗卵巢癌及预后提供依据。方法检测受试者TSGF、人附睾蛋白4(human epididymis secretory protein 4,HE4)、糖类抗原125(carbohydrate antigen 125,CA125)水平,并通过HE4与CA125的数值用卵巢癌风险性评估软件计算ROMA指数,统计分析卵巢癌患者血清TSGF、HE4、CA125水平和ROMA指数与临床分期之间的关系。结果与正常对照组和卵巢良性肿瘤组比较,卵巢癌组的TSGF、HE4、CAl25、ROMA指数水平和阳性率随着临床分期的升高而呈升高的趋势,绝经后均高于绝经前,差异均有统计学意义(P<0.05)。经相关性分析显示:ROMA指数阳性率与TSGF、HE4、CA125阳性率呈明显正相关(r=0.84;r=0.97;r=0.78)。卵巢癌组中ROMA指数的敏感性、特异性、阳性预测值、阴性预测值最高;CAl25的敏感性、特异性、阳性预测值、阴性预测值最低,两者差异均有统计学意义(P<0.05)。结论 ROMA指数的检测有助于评估患卵巢癌的风险性,对于早期卵巢癌具有更好的预测价值,可提高卵巢癌的早期诊断效率,进而提高五年生存率。Objective To evaluate the value of tumor specific growth factor (rSGF) and risk ovarian malignancy algorithm (RO- MA) in assessing the risk of ovarian cancer and analyse the relationship between the two indexes and clinical pathological stage of ovarian cancer for clinical diagnosis, treatment and prognosis of ovarian cancer. Methods The levels of TSGF, HE4 and CA 125 were detected in the subjects. ROMA index was calculated by using HE4 and CA125 numerical value of ovarian cancer risk assessment software. The relationship between serum levels of TSGF, HE4, CA125 and ROMA index in patients with ovarian cancer and clinical stage was analyzed statistically. Results Compared with normal control group and benign ovarian tumor group,the levels of TSGF, HE4, CA125, ROMA and the positive rate of ovarian cancer group increased with the clinical stage. All the parameters in postmenopause are higher than that in premenopause,with significant differences (P〈0.05). The positive rate of ROMA index was positively correlated with that of TSGF, HE4 and CA125 by correlation analysis. The sensitivity, specificity, positive predictive value and negative predictive value of ROMA index in ovarian cancer group were the highest;them of CA125 were the lowest, and the diffrences between ROMA index and CA125 were statistically significant(P〈0.05). Conclusion The detection of ROMA index helps to assess the risk of ovarian cancer and has better predictive value for early ovarian cancer, and it can improve the efficiency of early diagnosis of ovarian cancer and the five-year survival rate.
