摘  要:目的:通过观察艾灸疗法对克罗恩病(Crohn’s disease,CD)大鼠结肠组织神经营养因子GDNF、神经生长因子NGF及其受体p75表达的影响,探讨艾灸对克罗恩病大鼠肠神经系统的调节作用机理。方法:采用Morris的造模方法,将52只清洁级SD大鼠随机分为正常组、模型组、温和灸组、隔药灸组,每组12只,除了正常组大鼠外,其他均采用2,4,6-三硝基苯磺酸(trinitrobenzene sulfonic acid,TNBS)进行灌肠造模。选取双侧天枢穴进行温和灸和隔药灸治疗。采用HE染色光镜下观察各组大鼠结肠组织病理学变化,采用免疫组化法检测大鼠结肠组织的神经营养因子GDNF、NGF及其受体p75的表达。结果:与正常组相比,TNBS诱导大鼠结肠组织存在充血水肿、溃疡形成等组织病理学损伤,肠神经因子GDNF、NGF及其受体p75的表达均显著升高(P<0.05);温和灸和隔药灸治疗后大鼠结肠病理学损伤得到改善,肠神经因子GDNF、NGF及其受体p75的表达均有所下降(P<0.05)。结论:艾灸能下调CD大鼠肠神经因子GDNF和NGF及其受体p75的表达,提示对肠神经系统的调节可能是艾灸治疗克罗恩病的机制之一。Objective : To explore the regulation mechanism of moxibustion therapy in enteric nervous system of Crohn disease rats by observing the effect of moxibustion on neurotrophic factor GDNF, nerve growth factor NGF and its receptor p75. Methods: By using the modeling method of Morris, 52 clean level SD rats were randomly divided into normal group, model group, mild moxibustion group, herb -partitioned moxibustion (HPM) group, with 12 rats in each group. In addition to the normal group rats, others used TNBS enema model. Mild moxibustion and herb -partitioned moxibustion were applied on bilateral Tianshu( ST25 ). The histopathological variations of rats' colons were observed by adopting HE staining and expressions of neurotrophic factor GDNF, nerve growth factor NGF and its receptor p75 in the colon tissue of rats were examined by an immunohistochemical method. Results: Compared with the normal group,TNBS - induced rats' colon tissue presented congestion edema, ulceration histopathological injury and the expressions of GDNF, NGF and p75 increased significantly( P 〈 0.05). The pathological injury of rats' colon improved and the expressions of GDNF, NGF and p75 decreased by using the methods of mild moxibustion and herb - partitioned moxibustion( P 〈 0.05 ). Conclusion : Moxibustion can down - regulate the expressions of GDNF, NGF and p75 in the colon tissue of CD rats. One of the possible mechanism of moxibustion in the treatment of Crohn's disease is the regulation of the enteric nervous system.
