热毒宁注射液治疗特发性肺纤维化急性加重临床疗效回顾性观察

摘　　要：目的:回顾性观察热毒宁注射液治疗特发性肺纤维化急性加重临床疗效。方法:收集2013年1月—2015年12月30例特发性肺纤维化(IPF)住院患,应用热毒宁中药注射液患者14例为治疗组,未应用的16例为对照组,疗程为10 d的临床试验。观察两组治疗前后中医症状积分、临床疗效、血常规、血气分析及肺功能的改善情况。结果:1在中医证候评分和临床疗效方面,治疗组治疗后中医症状评分及总积分除喘促气短、发热、面赤均较对照组改善明显(P<0.05),总有效率较对照组明显好转;2在抗感染方面,治疗组治疗前后患者白细胞总数及中性粒细胞分类百分比,均较治疗前明显下降,其差异显著(P<0.05),与对照组治疗后中性粒细胞分类百分比具有统计学差异(P<0.05);3血气分析方面,治疗组治疗后血气分析Pa O2较对照组明显好转,其差异显著(P<0.05);4在肺功能改善方面,治疗组治疗后患者DLCO明显高于对照组,统计学上有显著差异(P<0.05)。结论:在IPF急性加重的治疗中,在西医基本治疗方案的基础上,辅以热毒宁注射液清热解毒化痰,能够明显改善患者临床症状,改善乏氧状态,提高弥散功能。Objective：To retrospectively observe the effect of Reduning Injection in treating acute idiopathic pulmonary fibrosis（IPF）. Method：Thirty cases in -patients who had been treated from Jan. 2013 to Dee. 2015 were collected in the clinical research. Fourteen patients were treated with Reduning Injection （tested group ） and the other 16 patients with conventional therapy（control group）. After 10 days＇ treatment,we observed the indexes such as TCM symptom score, clinical efficacy,blood routine,blood gas analysis and pulmonary function of two groups respectively. Result：l. The final TCM symptom score, aggregate score and total effective rate of tested group were significantly better than those of the con- trol group except for the index of palpitation shortness of breath, fever and flushed complexion （ P 〈 0.05 ）. 2. In the tested group, the total white blood ceils and the percent of neutrophile granulocyte were obviously decreased after treatment （ P 〈 0. 05）. And the final percent of neutrophile granulocyte of the tested group was significantly less than that of the control group（P 〈0. 05）. 3. In the aspect of blood gas analysis,the final PaO2 of tested group was obviously better than that of the control group（P 〈 0. 05）. 4. And compare with the control group,the final DLCO of tested group was also obviously better（P 〈 0. 05）. Conclusion：In the treatment of acute IPF,the integration of basic treatment of Western medicine and Reduning Injection which functions as clearing heat,removing toxicity and dispersing phlegm can effectively improve the clinical symptoms, hypoxia and pulmonary dispersion.
