滋水清肝饮加减联合前列地尔对肝肾阴虚型糖尿病周围神经病变患者外周血GSH、NF-κB及肝肾功能的影响

摘　　要：目的:滋水清肝饮加减联合前列地尔对肝肾阴虚型糖尿病周围神经病变(DPN)患者外周血还原性谷胱甘肽(GSH)、单个核细胞(NF-κB)及肝肾功能的影响。方法:将82例肝肾阴虚型DPN患者采用随机数字表法分为对照组和观察组,各41例。对照组给予前列地尔治疗,观察组在对照组基础上给予滋水清肝饮治疗,均以30 d为1个疗程,治疗2个疗程。评估临床疗效,测定正中神经、右腓肠神经的运动传到速度(MCV)、感觉传导速度(SCV),检测糖化血红蛋白(HbAlc)、还原性谷胱甘肽(GSH)、超氧化物歧化酶(SOD)、丙二醛(MDA)、单个核细胞(NF-κB)活性及肝肾功能指标。结果:观察组治疗后正中神经及右腓肠神经的MCV、SCV明显高于对照组(P<0.05);观察组治疗后GSH、SOD高于对照组,MDA低于对照组(P<0.05);观察组治疗后NF-κB、HbAlc低于对照组(P<0.05);两组治疗前后肝肾功能指标无差异(P>0.05)。结论:滋水清肝饮加减联合前列地尔治疗肝肾阴虚型DPN,可通过降低机体应激反应、抑制NF-κB活化发挥治疗作用,且对肝肾功能无影响。Objective：To observe the effects of Modified Zishui Qinggan Liquid combined with alprostadil on peripheral blood glutathione（GSH） and mononuclear cell（ NF - κB） and liver and kidney function of patients with diabetic peripheral neuropathy （DPN） and Yin deficiency of liver and kidney. Methods ：Eighty -two DPN patients with Yin deficiency of liver and kidney were divided by random number table into the control group and the observation group（41 cases in each ）. The control group received alprostadil treatment and the observation group added Zishui Qinggan Liquid on the ba- sis of the control group,lasting for 1 month as a course, a total of 2 courses of treatment. The clinical efficacy was as- sessed. The nervi medianus,right gastrocnemius nerve motor conduction velocity （MCV） and sensory conduction velocity were determined, hemoglobin （ HbAlc ）, glutathion （ GSH ） , superoxide dismutase （ SOD ） , malonaldehyde （ MDA）, mono- nuclear cell （ NF - κB） activity and liver and kidney function indexes were tested. Results ： MCV and SCV of nervi media- nus and right gastrocnemins nerve in the observation group after the treatment were significantly higher than those of the control group （ P 〈 0. 05 ）. GSH and SOD in the observation group after the treatment were significantly higher than those of the control group. MDA was significantly lower than that of the control group （P 〈 0. 05 ）. NF - KB and HbAlc in the observation group after the treatment were significantly lower than those of the control group （ P 〈 0. 05 ）. There was no significant difference in liver and kidney function indexes before and after the treatment in the two groups （P 〉 0. 05 ）. Conclusion ： Modified Zishui Qinggan Liquid combined with alprostadil in treating DPN patients with Yin deficiency of liver and kidney plays a therapeutic role by reducing body stress reaction and inhibiting NF - κB activation and has no effect on liver and kidney function.
