摘 要:目的观察不同糖代谢人群血清25-(OH)D_3水平的变化,并探讨25-(OH)D_3与糖代谢紊乱相关指标及胰岛素抵抗的相关性。方法收集我院2015年1月至2016年3月患者的临床资料,根据糖尿病诊断标准分为:正常对照组(对照组)、糖耐量异常组(IGT组)、2型糖尿病组(T2DM组),观察各组的血清25-(OH)D_3水平及其他相关指标的变化,并分析25-(OH)D_3水平与BMI、FPG、GHbA1C、FINS、HOMA-IR、HOMA-IS的相关性,探讨其在糖代谢过程中的作用。结果 3组间的25(OH)D_3水平差异有统计学意义(P<0.01);男、女之间体质量指数差异有统计学意义(P<0.01),而不同性别间的FPG、GHbA1C、FINS、HOMA-IR、HOMA-IS、25-(OH)D_3差异均无统计学意义(P>0.05);25(OH)D_3与FINS、HOMA-IS呈正相关(P<0.01),与FPG和GHbA1C呈负相关(P<0.01),与BMI、HOMA-IR无相关性(P>0.05)。结论糖耐量异常者及2型糖尿病患者血清维生素D水平低于正常人群,低水平的血清25-(OH)D_3可能影响人体的胰岛素分泌功能,从而引起血糖增高。Objective To observe the level of 25-(OH)D3among people with different glucose metabolic abilities and discuss its correlation with indicators of glucose metabolic disorder as well as insulin resistance.Methods This paper collected clinical data of our hospital from January 2015 to March 2016 and divided the patients into three groups according to diagnostic criteria for diabetes:control group,IGT group and T2 DM group.Then we focused on observing the level of 25-(OH)D3and other indicators among different groups,analyzed the correlation of 25-(OH)D3 with BMI,FPG,GHbA1 C,FINS and HOMA-IS,and investigated the function of 25-(OH)D3in the process of glucose metabolism.Results The difference of 25-(OH)D3level in the three groups was significant(P〈0.01);The difference of BMI between male and female was significant(P〈0.01),while the differences of GHbA1 C,FINS,HOMA-IR,HOMA-IS,and 25-(OH)D3in different sex were not significant(P〈0.05);25(OH)D3was positively correlated with FINS and HOMA-IS(P〈0.01),while it was negatively correlated with FPG and GHbA1C(P〈0.01);25(OH)D3had no relation with BMI and HOMA-IR(P〉0.05).Conclusion Serum vitamin D levels of IGT and T2 DM groups were lower than that of the control group.The low level of serum 25-(OH)D3 might inhibit insulin secretion,and lead to the raise of blood glucose.
