摘　　要：目的探讨超低出生体重儿(ELBWI)的临床特点及疗效分析。方法运用回顾性分析方法对2010年11月至2016年9月某院新生儿重症监护室收治的215例ELBWI的临床资料。结果 215例患儿救治存活率44.2%(95/215),病死率55.8%(120/215),放弃治疗率23.2%(50/215)。主要并发症为呼吸暂停、新生儿呼吸窘迫综合征(NRDS)及病理性黄疸。120例病死病例中,有50例因放弃治疗,其他原因为40例新生儿呼吸窘迫综合征,15例败血症,5例坏死性小肠结肠炎,5例肺出血,5例颅内出血。不同出生体重组、胎龄组病死率比较差异均有统计学意义(χ2=32.755,20.289,P<0.05)。结论ELBWI住院期间存在较高的并发症发生率和较低的存活率,治疗难度较大,病死原因与患者家属放弃治疗有一定的关系,各种并发症的发生与多种影响因素有关,临床上需综合防治。Objective To analyze the clinical features and efficacy of extremely low birth weight infants（ELBWI）.MethodsRetrospective analysis was conducted using clinical data of ELBWI admitted in our hospital between November 2010 and September 2016.Results Rate of survival in 215 cases was 44.2%（95/215）,the mortality was 55.8%（120/215）,and treatment was abandoned in 23.2%（50/215）.The main complications were apnea,neonatal respiratory distress syndrome（NRDS）and pathological jaundice.Among the 120 ELBWI deceased,causes that led to mortality were abandon of treatment（50cases）,NRDS（40cases）,septicemia（15cases）,necrotizing enterocolitis（5case）,pulmonary hemorrhage（5case）,and intracranial hemorrhage（5case）.There were significant differences in the mortality between different birth weight groups and gestational age groups（χ^2=32.755,20.289,P〈0.05）.Conclusion ELBWI have many severe complications and low survial rates.There is a certain relationship between the mortality and the abandon of treatment.The morbidities could be influenced by many factors which should be managed comprehensively.
