去痛片致紫癜型药疹1例

摘　　要：患者女,72岁,服用去痛片后双下肢瘀点、瘀斑伴痒7d。皮肤科情况:双下肢见散在大小不等瘀点、瘀斑,呈紫红色,部分瘀斑上可见血疱,无破溃及渗出,压之不退色,无压痛,未见关节肿胀。皮损组织病理示:表皮细胞内水肿,部分角质细胞坏死形成表皮内疱,局灶性基底细胞液化变性,真皮浅层水肿,大量血管外红细胞,真皮血管周围见淋巴细胞及多量嗜酸性粒细胞浸润。结合临床与病理诊断为紫癜型药疹。A 72yearold female presented with petechia and ecchymosis with pruritus on her bilateral lower extremities after medication for a weekCutaneous examination revealed that a large number of mauve petechia and ecchymoses on her lower extremities, with hemophysallisHistopathology showed epidermal intracellular edema, blister within the epidermis, liquefaction degeneration of basal layer, numerous erythrocytes in the upper dermis, few lymphocytes and numerous eosinophils perivascular infiltration in the dermisThe final diagnosis was purpuralike drug eruption
