摘 要:目的观察丙泊酚对肝脏缺血再灌注损伤患者外周血中性粒细胞黏附因子CD11b表达的影响,探讨丙泊酚对肝脏缺血再灌注损伤的保护作用。方法选取2013年1月至2015年6月黄冈市中心医院收治的择期行阻断肝门肝脏手术患者110例,根据麻醉方法分为观察组和对照组,每组55例。对照组患者给予咪达唑仑、芬太尼和阿曲库铵麻醉,观察组患者在对照组基础上给予丙泊酚麻醉;分别于肝门开放前及开放10、30 min和开放1、2 h时检测患者外周血CD11b的表达;并于肝门开放前、开放30 min、开放1 h时取正常肝组织,测定细胞间黏附因子-1(ICAM-1)mRNA表达。结果观察组和对照组患者肝门阻断时间分别为(18.13±0.23)、(20.21±1.09)min,术中出血量分别为(824.52±21.52)、(832.61±19.59)m L,2组患者肝门阻断时间及术中出血量比较差异均无统计学意义(P>0.05)。肝门开放前2组患者外周血CD11b表达比较差异无统计学意义(P>0.05)。肝门开放10、30 min和1、2 h时2组患者外周血CD11b表达显著高于肝门开放前(P<0.05)。肝门开放10、30 min时2组患者外周血CD11b表达比较差异无统计学意义(P>0.05)。肝门开放1、2 h时观察组患者外周血CD11b表达显著低于对照组(P<0.05)。肝门开放前2组患者肝组织中ICAM-1 mRNA表达比较差异无统计学意义(P>0.05)。肝门开放30 min和1 h时,2组患者肝组织中ICAM-1 mRNA表达均显著高于肝门开放前(P<0.05),且观察组患者肝组织中ICAM-1 mRNA表达显著低于对照组(P<0.05)。结论丙泊酚能有效降低肝脏缺血再灌注损伤患者外周血CD11b的表达,对肝脏缺血再灌注损伤有一定的保护作用。Objective To investigate the protective effect of propofol on hepatic ischemia-reperfusion injury by observing the effect of propofol on the expression of neutrophil adhesion factor CD11 b in peripheral blood of patients with hepatic ischemia-reperfusion injury. Methods A total of 110 patients who underwent selective operation on liver under hepatic portal occlusion were selected in Huanggang Central Hospital from January 2013 to June 2015. The patients were divided into observation group and control group according to anesthetic method,55 patients in each group. The patients in the control group were anesthetized with midazolam,fentanyl and atracurium; the patients in the observation group were anesthetized with propofol. The CD11 b expression in peripheral blood was detected at the time points of before portal opening and 10,30 min,1,2 h after portal opening. The intercellular cell adhesion molecule-1( ICAM-1) mRNA expression in normal hepatic tissue was detected at the time points of before portal opening and 30 min,1 h after portal opening. Results The portal occlusion time in the observation group and control group was( 18. 13 ± 0. 23) and( 20. 21 ± 1. 09) min,and the intraoperative blood loss volume was( 824. 52 ± 21. 52) and( 832. 61 ± 19. 59) m L respectively,there was no significant difference in them between the two groups( P 0. 05). There was no significant difference in the expression of CD11 b in peripheral blood between the two groups before portal opening( P 0. 05). The expression of CD11 b in peripheral blood at the time points of 10 and 30 min,1 and 2 h after portal opening was significantly higher than that before portal opening in the two groups( P 0. 05). There was no significant difference in the expression of CD11 b in peripheral blood between the two groups at the time point of 10,30 min after portal opening( P 0. 05). The expression of CD11 b in peripheral blood in observation group was significantly lower than that in control group at the time point
