摘　　要：目的评价兴国县农村居民慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)防治一体化管理效果。方法对兴国县龙口镇、社富乡、梅窖镇、高兴镇、均村乡、城岗乡6个试点乡镇18个村的933例COPD农村患者,实施1年的防治一体化综合干预、随访和管理,比较干预前后的效果。结果与干预前比较,干预后患者建档率和规范化管理率提高(99.46%比36.50%,83.41%比12.72%,均P<0.01),干预后COPD患者急性加重率降低(5.35%比11.63%,P<0.01),COPD知晓率提高(96.72%比19.60%,P<0.01)。结论实施集"预防-诊疗-保健-健康教育-管理"为一体的COPD综合干预,可有效控制COPD患者的相关危险因素,减少急性加重的发生、提高患者COPD知识的知晓率,减少并发症,是控制COPD的有效措施。Objective To evaluate the efficacy of integrated management of chronic obstructive pulmonary disease(COPD)prevention and treatment in rural residents of Xingguo county.Methods A total of 933 COPD residents from 18 villages of 6pilot townships(Longkou,Shefu,Meijiao,Gaoxing,Juncun and Chenggang)were selected in this study.The efficacy of comprehensive intervention,follow-up and management integration was evaluated after 1year of implementation.Results The filing rate,standardized management rate and COPD awareness rate after intervention were higher than those before intervention(99.46% vs 36.50%,83.41% vs 12.72% and96.72% vs 19.60%,respectively;P<0.01).The incidence of acute exacerbation of COPD after intervention was lower than that before intervention(5.35% vs 11.63%,P<0.01).Conclusion The implementation of comprehensive prevention-treatment-health care-health education-management can effectively control the related risk factors,reduce the incidence of acute exacerbation,increase the awareness and reduce the complications in patients with COPD.Therefore,the integrated management of prevention and treatment of COPD is an effective measure to control COPD.
