摘　　要：目的:评价3.0T MRI半剂量Gd-DTPA动态增强技术在检查儿童垂体下丘脑病变中的应用价值。材料与方法:回顾性分析84例临床怀疑下丘脑垂体病变并且进行了半剂量Gd-DTPA动态增强磁共振扫描的患儿,两位高年资医师对图像质量进行了评价。结果:3.0T MRI半剂量Gd-DTPA动态增强扫描在垂体下丘脑扫描检查的成功率为100%。除4例患儿稍有运动伪影外,其余80例图像质量均能满足临床诊断。正常垂体32例,青春期垂体42例;垂体Rathke囊肿2例,下丘脑灰结节错构瘤2例,生殖细胞瘤1例,垂体柄小结节1例,垂体微腺瘤4例。所有病例诊断均与临床随访诊断结果一致。结论:儿童垂体半剂量Gd-DTPA动态增强磁共振扫描是可行的,可以作为儿童青春期垂体病变筛查的常规影像学检查方法。Purpose： To evaluate the application value of 3.0T MRI using dynamic contrast enhancement magnetic resonance imaging （DCE-MRI） with a half dose of Gd-DTPA in the examination of pituitary hypothalamic lesions in children. Materials and Methods： We retrospectively analyzed 84 cases, who all performed DCE-MRI using a half dose of Gd-DTPA. Two experienced radiologists evaluated the quality of all images. Results： All DCE-MRI for pituitary hypothalamus scans were successfully obtained with a half dose of Gd-DTPA. Other than 4 cases with slight motion artifacts, the remaining 80 patients were successfully applied for the clinical diagnosis. In which, there were 32 cases with normal MRI appearance of pituitary gland, 42 cases with pubertal pituitary gland, 2 cases with Rathke＇ s cysts, 2 cases with hypothalamic hamartoma of the tuber cinereum, 1 case with germ cell tumor, 1 case with a nodular lesion in the pituitary stalk, and 4 cases with pituitary adenoma. All diagnoses for 84 cases were consistent with the results of clinical follow-up. Conclusion： DCE-MRI of the pituitary gland using a half dose of Gd-DTPA is feasible in children and can be used as a routine imaging examination for the screening of pituitary lesions in children.
