B-flow技术在诊断骨筋膜室综合征中的应用

摘  要:目的:探讨B-flow技术在骨筋膜室综合征中的显像特点及其诊断价值。材料与方法:回顾分析7例临床疑诊为骨筋膜室综合征患者资料,先常规观察患侧小腿肿胀程度、肌肉回声及血运情况,并测量足背动脉血流峰值,然后应用B-flow技术仔细观察患侧小腿各筋膜室内肌肉的血流显像特征,总结并分析B-flow显像特征与手术结果的相关性。结果:7例临床疑诊病例中,发生在前侧骨筋膜室5例,后侧骨筋膜室2例。CDFI显示病变骨筋膜室内肌肉血流信号存在1例,病变骨筋膜室内肌肉血流信号消失6例,血流敏感性为14.2%(1/7);B-flow显示病变骨筋膜室内肌肉血流信号存在3例,病变骨筋膜室内肌肉血流信号消失4例,血流敏感性为42.9%(3/7),两组血流敏感性比较,差异有统计学意义(p<0.05)。4例B-flow显示病变骨筋膜室内肌肉血流信号消失,经临床证实骨筋膜室内压力呈不同程度增高,其中有3例存在肌组织缺血坏死现象,特异性为75.0%(3/4),这说明B-flow显示病变骨筋膜室内肌肉血流信号消失与肌组织缺血坏死呈良好的相关性。结论:B-flow技术不仅能客观反映骨筋膜室内肌肉血流情况,而且能间接判断骨筋膜室内压力,为临床诊断和治疗提供重要的参考信息。Purpose: To study the characteristics and diagnostic value of imaging in B-flow technique in bone compartment syndrome. Materials and Methods: Retrospective analysis of seven patients data in clinically suspected compartment syndrome. The conventional observation ipsilateral leg swelling, muscle echo and blood supply, and to measure the dorsal artery peak, and then apply the B-flow technique carefully observed the calf muscle of the respective interior compartment flow imaging features. Summarize and analyze the relevance between B-flow imaging characteristics and surgical results. Resuits: There are the compartment front side in 5 cases and 2 cases of bone the rear compartment in the clinically suspected ca- ses. CDFI showed lesions osteofascial indoor muscle blood flow signals the presence of one case, the lesion Osteofascial indoor muscle blood flow signals disappeared in 6 cases, blood flow sensitivity was 14. 2% (1/7) ; B-flow showed lesions Osteofascial indoor muscle blood flow signal is present in 3 cases, lesions osteofascial indoor muscle blood flow signals disappeared in 4 cases, blood flow sensitivity was 42.9% (3/7), the sensitivity of blood groups, the difference was statistically significant (p 〈 0. 05). 4 cases of B-flow showed lesions osteofascial indoor muscle blood flow signal disappears, clinically proven bone fascia chamber pressure was increased to varying degrees, including three cases the presence of muscle tissue necrosis phenomenon, a specificity of 75.0% (3/4). This shows the crucial relevance between lesions osteofascial indoor muscle blood flow and muscle tissue necrosis. Conclusion: B-flow technology not only reflect objectively osteofascial indoor muscle blood flow but also determine indirectly osteofascial chamber pressure. It provide important reference information for clinical diagnosis and treatment.
