摘  要:目的探讨儿童早期经历创伤后压力心理障碍症(PTSD)与儿童后期的智力发育和认知能力的影响。方法收集80例早期经历过PTSD的儿童作为研究组,依据病情严重程度分为重PTSD组和轻PTSD组,以教育程度相似、年龄匹配的70例健康儿童作为对照组。结果重PTSD组、轻PTSD组和对照组之间在教育程度、年龄方面比较,差异无统计学意义(P>0.05)。根据《韦氏儿童智力量表》的统计,3组智力水平有统计学差异(P<0.05),重PTSD患者组的分数明显低于对照组(P<0.05)。根据"威斯康星卡片分类测验"的统计,轻、重PTSD组的持续性错误数、正确应答数、完成分类数、总应答数成绩均差于对照组(P<0.01)。结论儿童早期经历PTSD严重的患者后期的智力明显下降,儿童早期经历PTSD的患者后期的认知能力较差。Objective To explore the effect of post-traumatic stress disorder (PTSD) in early childhood on intelligence development and cognitive competence in the later stage of children. Methods Eighty children with PTSD in early childhood were collected as study group, then they were divided into severe PTSD group and mild PTSD group according to the severity of the disease. Seventy healthy children with similar educational level and age were selected as control group. Results There was no statistically significant difference in educational level and age among the three groups ( P〉0.05 ) . There was statistically significant difference in intelligence level among the three groups ( P〈0. 05 ), the score of Wechsler Intelligence Scale for Children in severe PTSD group was statistically significantly lower than that in control group (P〈0. 05 ) . According to Wisconsin Card Sorting Test (WCST), the numbers of preservative error, correct responses, categories completed and the total numbers of responses in mild PTSD group and severe PTSD group were statistically significantly worse than those in control group (P〈0. 01 ) . Conclusion The intelligence level in the later stage of children with PTSD in early childhood decreases signifi- cantly, and the cognitive competence is poor.
