摘 要:目的探讨高压氧联合rh-EPO对窒息新生大鼠心肌细胞Bcl-2/Bax表达的影响,为临床治疗新生儿窒息后心肌损害提供分子理论依据。方法选取新生3~7 d的Wistar大鼠40只为研究对象,随机分为对照组、窒息组、rh-EPO组、高压氧组、高压氧联合rh-EPO组各8只,分别给予相应的干预措施。病理学检查及半定量分析常规HE染色,光镜下观察心肌病理改变。对各组心肌细胞采用免疫组织化学染色法检测Bcl-2/Bax蛋白表达。结果新生大鼠心肌细胞病理学观察显示:高压氧联合rh-EPO组心肌细胞结构基本正常,细胞界限欠清晰,心肌纤维排列基本正常;Bcl-2/Bax蛋白表达阳性细胞胞浆呈棕黄色染色。窒息组阳性细胞最多,高压氧联合rh-EPO组阳性细胞较少。结论新生大鼠窒息后采用高压氧与rh-EPO治疗,两者产生协同作用,通过增加Bcl-2表达、减少Bax表达而发挥心肌保护作用,Bcl-2/Bax比值明显增加。Objective To explore the effect of hyperbaric oxygen combined with recombinant human erytbropoietin (rh-EPO) on Bcl- 2/Bax expression in myocardial cells of neonatal rats with asphyxia, provide a theoretical basis for clinical treatment of neonatal myocardial damage after asphyxia. Methods Forty neonatal Wistar rats aged 3-7 days were selected and randomly divided into control group, asphyxia group, rh-EPO group, hyperbaric oxygen group, hyperbaric oxygen combined with rh-EPO group, 8 rats in each group. The rats in different groups were treated by corresponding intervention measures. Pathological examination and semi-quantitative analysis, conventional HE staining, light microscope observation of myocardial pathological changes were perfomred. The expressions of Bcl-2/Bax protein in myocardial ceils of neonatal rats in different groups were detected by immunohistochemical staining. Results Pathological results of myocardial cells in neonatal rats showed that the structure of myocardial cells in hyperbaric oxygen combined with rh-EPO group was normal, the cell line was not clear, and the arrangement of myocardial fibers was normal. The cytoplasm of myocardial cells with positive expression of Bcl-2/Bax protein was brown yellow. The positive cells in asphyxia group were the highest, and the positive cells in hyperbaric oxygen combined with rh- EPO group were less. Conclusion Hyperbaric oxygen and rh-EPO have synergistic effect in treatment of neonatal rats with asphyxia, which can protect myocardial cells by up-regualtion of Bcl-2 expression and down-regulation of Bax expression, the ratio of Bcl-2 to Bax increases significantly.
