羊膜制品治疗重度宫腔粘连患者的效果

摘　　要：目的观察羊膜制品在重度宫腔粘连中的临床治疗效果。方法选取重度宫腔粘连患者100例,根据治疗方案不同分为对照组和观察组各50例。对照组采用Foley球囊治疗,观察组采用羊膜制品治疗,比较两组治疗效果。结果两组手术前宫腔粘连评分及月经量评分差异无统计学意义(P>0.05);观察组手术后宫腔粘连评分显著低于对照组(P<0.05);观察组手术后月经量评分显著高于对照组(P<0.05);两组治疗前子宫内膜厚度差异无统计学意义(P>0.05);观察组术后3个月、术后6个月子宫内膜厚度显著大于对照组(P<0.05);观察组术后6个月宫腔再粘连发生率为18.00%,显著低于对照组的28.00%(P<0.05)。结论与Foley球囊治疗相比,重度宫腔粘连患者采用羊膜制品治疗效果更理想,能改善月经量,降低术后粘连评分,值得推广应用。Objective To observe the clinical effect of amniotic membrane products in treatment of patients with severe intrauterine adhesion. Methods One hundred patients with severe intrauterine adhesion were selected and divided into control group and observation group according to different treatment options, 50 patients in each group. The patients in control group were treated by Foley balloon therapy, and the patients in observation group were treated by amniotic membrane products. The curative effects in the two groups were compared. Results Before treatment, there was no statistically significant difference in intrauterine adhesion score and menstrual blood volume score between the two groups （P〉0. 05 ） . After treatment, intrauterine adhesion score in observation group was statistically significantly lower than that in control group （P〈0.05） ; menstrual blood volume score in observation group was statistically significantly higher than that in control group （P〈0. 05） . Before treatment, there was no statistically significant difference in endometrial thickness between the two groups （ P〉0.05 ） . At three and six months after treatment, endometrial thicknesses in observation group were statistically significantly thicker than those in control group （P〈0. 05 ） . The recurrence rate of intrauterine adhesion at six months after treatment in observation group was 18. 00%, which was statistically significantly lower than that in control group （28.00%） （P〈0. 05） . Conclusion Compared with Foley balloon therapy, the effect of amniotic membrane products in treatment of patients with severe intrauterine adhesion is better, amniotie membrane products can improve menstrual blood volume and reduce postoperative adhesion score, which is worthy of promotion and application.
