颅骨缺损金属钛网修补术后感染的相关因素分析

摘　　要：目的探讨颅骨缺损金属钛网修补术后发生切口感染的原因。方法 92例颅骨缺损修补术患者分别按年龄、性别、受伤原因、修补时机、钛网类型、是否二次切口、缺损面积、手术时间以及是否切口愈合不良进行分组。统计各组发生切口感染的例数,对数据应用统计学方法进行处理。结果不同年龄、性别、受伤原因、修补时机、钛网类型、是否二次切口、缺损面积、手术时间组的感染发生率的差异均无统计学意义。而术后出现切口愈合不良组的感染发生率显著高于切口愈合正常组,差异有统计学意义。结论颅骨修补术后感染与切口愈合情况有密切关系,术前需评估切口的选择,尽量避免出现血供差的情况。术中分离皮瓣避免菲薄并出现发根及脂肪层。术后需密切注意切口愈合情况,及时换药观察切口很重要。Objective To explore the causes of incision infection of skull defect cranioplasty with Titanium mesh.Methods 92 patients who underwent cranioplasty with titanium mesh were analyzed according to age,gender,cause of decompressive craniectomy,time between decompressive craniectomy and cranioplasty,type of titanium mesh,whether the incision was cut twice,skull defect area,operation duration,and the wound healing poorly. The number of incision infection cases of each subgroup had been analyzed. The data had been processed by statistical method. Results All of age,gender,cause of decompressive craniectomy,time between decom-pressive craniectomy and cranioplasty,type of titanium mesh,whether the incision was cut twice,skull defect area,operation duration had no significant difference with incidence of incision infection. The incidence of incision infection in poorly wound healing group was higher than it was in normal wound healing group. There was significant difference. Conclusion Infection after cranioplasty is closely related to wound healing. To avoid the poor blood supply situation,we evaluate the choice of incision preoperatively.When we separated the flap,the emergence of hair root and fat layer were forbidden. We should pay more attention to wound healing postoperatively,and timely observing the incision is very important.
