从黄芩滑石汤探讨利湿通窍法对脾胃湿热证型慢性鼻窦炎术后疗效

摘　　要：目的:观察利湿通窍法对脾胃湿热型慢性鼻窦炎术后的临床疗效,为中药汤剂的应用提供临床依据。方法:慢性鼻窦炎脾胃湿热证患者72例,随机分为试验组和对照组,每组36例。两组均行功能性鼻内镜手术,观察术后6 w和8 w患者症状的视觉模拟量表(VAS)评分和术腔黏膜形态Lund-Kennedy评分,观察术前及术后8 w副鼻窦CT LundMackay评分,并进行临床疗效分析。结果:两组术后6 w及8 w VAS评分、术腔黏膜形态Lund-Kennedy评分比较差异有统计学意义(P<0.01);两组治疗前后副鼻窦CT Lund-Mackay评分、总有效率比较差异有统计学意义(P<0.05)。结论:脾胃湿热证慢性鼻窦炎术后患者运用利湿通窍法进行中医辅助治疗,可改善症状,对术后术腔黏膜形态的恢复具有促进作用,值得临床进一步推广及开发。Objective：To observe the effect of Huangqin Huashi decoction on patients with chronic sinusitis（damp-heat syndrome of spleen-stomach）after surgery,to provide clinical basis for the application of the decoction of Chinese medicine. Methods：72 cases were randomly divided into experimental group and control group（each 36 cases）. The patients in two groups were treated with functional endoscopic sinus surgery. Huangqin Huashi decoction was given to patients in experimental group in oral. After 6 weeks and 8 weeks,symptoms of visual analogue scale（VAS）and the operative cavity mucosal morphology of Lund-Kennedy score were observed;in the preoperative and 8 weeks after surgery,CT Lund-Mackay score of paranasal sinus was evaluated. The clinical curative effect was analyzed. Results：The differences of the VAS score and the Lund- Kennedy score were statistically significant between two groups after 6 weeks and 8 weeks operation（P〈0.01）. Lund-Mackay scores and total efficiency of two groups had statistically difference（P〈0.05,P〈0.01）. Conclusion：Huangqin Huashi decoction can improve symptoms,promote novation of mucous membrane in chronic sinusitis patients with damp-heat syndrome of spleen-stomach after functional endoscopic sinus surgery. It is worthy of further promotion in clinic.
