膝关节骨性关节炎的针灸治疗选穴特点及其分析

摘　　要：目的:通过总结膝关节骨性关节炎针灸选穴特点,分析膝关节骨性关节炎的病因病机及诊疗特点,从而更好地指导临床,为以后膝关节骨性关节炎的治疗及预防打下坚实的基础。方法:主要运用文献检索的方式,对近十年来针灸治疗膝关节骨性关节炎临床报道文献进行整理,并结合实际临床观察,研究膝关节骨性关节炎的病因病机及诊疗方案。结果:共纳入137篇文献,涉及的腧穴及阳性刺激点共61个,取穴总频数为937次,涉及的十四经共10条,其中足太阳膀胱经腧穴数目最多,足阳明胃经的取穴频数最多;分布部位以下肢膝关节周围最多。结论:针灸治疗膝骨性关节炎选穴时以循经取穴和局部取穴为原则,重在起到补益肝肾气血,祛除风寒湿邪以及通络止痛的作用。Objective：Through summarizing the characteristic of acupuncture points of knee osteoarthritis,to analyze etiology and pathogenesis,diagnostic and therapeutic features of knee osteoarthritis,so as to guide clinical well. Methods：Mainly with the method of literature retrieval,nearly a decade of clinical literature reports about knee osteoarthritis treated with acupuncture were collected. The etiology and pathogenesis,diagnosis and treatment plan of knee osteoarthritis were dicussed. Results：137 papers were involved.The research showed that acupoints and positive stimulate points was 61,total frequency of acupoints was 937. And ten meridians were involved. The characters of acupuncture points of knee osteoarthritis as follow：the acupoints amount of Bladder meridian was the most and acupoints of stomach meridian of foot-yangming were selected most frequently;most of acupoints were along low limbs around the knee joint. Conclusion：The principle of selecting points about knee osteoarthritis treated with acupuncture is along meridians and local point selection. And the role is to nourish the liver and kidney Qi,dispelling wind-cold damp pathogen and activating meridians to stop pain.
