特重度烧伤患者急诊ICU救治早期真实体验的质性研究

摘　　要：目的:探讨特重度烧伤患者急诊ICU救治早期的真实体验。方法:采用现象学方法,以目的抽样法抽取爆炸事故后9例急诊ICU救治的特重度烧伤患者进行深度访谈。结果:特重度烧伤患者急诊ICU救治早期的真实体验体现在:对生命的热爱、安全感的缺失以及对重生的迷惘3个主题。结论:护理人员应关注此阶段患者体验,在积极给予患者生命支持的同时应注重包括非现实乐观主义的早期植入与延续、改善ICU环境、加强疼痛管理及积极有效的信息支持等人文主义关怀护理措施的应用,对减轻患者创伤后的应激障碍,促进创伤后成长具有现实性意义。Objective： To discuss the real experience of the extremely severe burn patients in early stage of ICU of emergency treatment. Methods： Phenomenological method was taken,and 9 cases of extremely severe burn patients receiving emergency treatment in ICU after explosion accidents were sampled with purposeful sampling method to receive in- depth interview. Results： The real experience of the extremely severe burn patients in early stage of ICU of emergency treatment was embodied 3 topics,including the love of life,lack of security,and confusion of rebirth. Conclusion： The nursing personnel should pay attention to the experience of patients in this stage,and in addition to active provision of life support to the patients,it is ineluded promote the early implantation and continuation of unrealistic optimism,improve the ICU environment,and strengthen the pain management and active and effective information support,as well as the application of nursing measures based on humanistic care,which was of realistic significance to alleviate the patients＇ post- traumatic stress disorder and promote their post- traumatic growth.
