系统心理护理对超声引导导丝定位乳腺微小病变切除患者手术时间及负性情绪的影响

摘　　要：目的：探讨心理护理对超声引导导丝定位乳腺微小病变切除患者手术时间及负性情绪的影响。方法：选取我院2015年2月~2016年5月临床触诊阴性而有异常影像学表现的乳腺微小病变患者60例,随机等分为观察组和对照组,所有患者均行乳腺超声引导导丝定位穿刺切除,观察组实施常规护理与系统心理护理相配合,而对照组实施常规护理,统计分析穿刺及切除成功率,分析比较两组的手术时间,并采用视觉模拟量表（VAS）、Zung焦虑自评量表（SAS）及症状自评量表（SCL-90）对患者的负性情绪进行调查。结果：观察组患者均一次性完成穿刺,观察组切除成功率较对照组成功率略高,但差异无统计学意义（P〉0.05）;观察组手术时间与对照组的手术时间相比缩短（P〈0.05）;观察组术后VAS、SAS及SCL-90评分均优于对照组（P〈0.05）。结论：系统心理护理配合超声引导导丝定位乳腺微小病变切除,可以有效的缓解患者的负性情绪,顺利完成穿刺切除,缩短手术时间,值得临床上采用。Objective： To discuss the influence of systemic psychological nursing on time of operation and negative emotion of the patients of minute breast lesion resection via wire- localization guided by ultrasound. Methods： Selected 60 cases of patients with minute breast lesions showing negative and abnormal imaging findings clinically diagnosed in the period from February 2015 to May 2016 were selected and equally divided into observation group and control group at random,and all patients received breast lesion puncturing resection via wire- localization guided by ultrasound. The,the patients in the observation group received the routine nursing combined with systemic psychological nursing,and the patients in the control group received routine nursing. Puncture and resection success rate was statistically analyzed,and the two groups were analyzed and compared in operation time,and also the patients＇ negative emotion was investigated by use of visual analogue scale（ VAS）,self- rating anxiety scale（ SAS） of Zung,and symptom checklist（ SCL- 90）. Results：The patients from observation group all completed puncture surgery in one time,and the observation group had a higher resection than the control group,but the difference was not statistically significant（ P〈0. 05）; the observation group had a shorter operation time than the control group（ P〈0. 05）; and the observation was superior to the control group in scoring of VAS,SAS and SCL- 90 after operation. Conclusion： Through the combination of systemic psychological nursing and minute breast lesion resection via wire- localization guided by ultrasound,the negative emotion of patients could be effectively eased,the puncturing resection could be completed smoothly,and the operation time could be shortened,so it was worthy of application in clinical practice.
