3种术式治疗重度内痔疗效对比研究

摘　　要：目的观察选择性痔上黏膜吻合术(TST)、吻合器痔上黏膜环切术(PPH)、外剥内扎术(MilliganMorgan)3种术式治疗Ⅲ,Ⅳ度内痔的临床疗效。方法将285例Ⅲ,Ⅳ度内痔患者随机分为A,B,C 3组,A组97例在常规治疗的基础上采用TST治疗,B组89例在常规治疗的基础上采用PPH治疗,C组99例在常规治疗的基础上采用Milligan-Morgan治疗。术后1月观察3组的临床疗效、术后并发症发生率、手术时间、术后疼痛时间、创面愈合时间及住院时间。结果 A组痊愈率为95.9%,明显高于B组(92.1%)和C组(80.8%),差异均有统计意义(P<0.05);A组术后并发症发生率明显低于B组和C组,手术时间、术后疼痛时间、创面愈合时间及住院时间均明显短于B组和C组,差异均有统计意义(P<0.05或P<0.01)。结论 TST治疗Ⅲ,Ⅳ度内痔临床疗效显著,可明显减少术后并发症的发生,缩短手术时间、术后疼痛时间、创面愈合时间及住院时间。Objective To make a comparative observation on severe internal hemorrhoid treated by tissue-selecting therapystapler（ TST）,patent prosecution highway（ PPH） and Milligan-Morgan respectively. Methods285 patients with severe internal hemorrhoid were randomly divided into Group A（ 97 cases）,Group B（ 89 cases）and Group C（ 99 cases）. Besides conventional therapy,Group A was treated with TST,Group B with PPH,while Group C with Milligan-Morgan. One month after operation,clinical effect,postoperative complication,operation time,postoperative pain time,wound healing time and length of stay were compared and observed. Results The cure rates of three groups were 95.9%,92. 1% and 80. 8% respectively,the difference was significant（ P 〈 0. 05）.Postoperative complication of group A was obviously lower than group B and C as well as the operation time,postoperative pain time,wound healing time and length of stay of group A were all shorter than that in other two groups,the differences were statistically significant（ P〈0.05 or P 〈0.01）. Conclusion The operative method of TST can lower the incidence of complications,shorten operation time,postoperative pain time,wound healing time and length of stay. TST has a distinct effect in treating internal hemorrhoid.
