不同剖宫产缝合方式对子宫切口瘢痕憩室形成的影响

摘　　要：目的探讨不同剖宫产缝合方式对子宫切口瘢痕憩室形成的影响。方法选取2014年3月至2015年9月东莞市第五人民医院收治的365例择期行剖宫产产妇作为研究对象,按随机数字表法将其分为对照组和研究组。研究组产妇剖宫产切口采用双层连续缝合,对照组产妇剖宫产切口采用单层连续缝合。比较两组产妇憩室肌层厚度、憩室形成例数、憩室容积、术中出血量、手术时间、肛门排气时间、术后恶露持续时间及住院时间。结果两组术中出血量、手术时间、肛门排气时间、术后恶露持续时间及住院时间比较差异无统计学意义(P>0.05)。研究组产妇憩室形成率(3.83%)与憩室容积[(0.36±0.12)mL]低于对照组[9.34%,(0.49±0.09)mL],憩室肌层厚度[(7.63±1.92)mm]高于对照组[(5.48±2.51)mm],差异均有统计学意义(P<0.05)。结论双层连续缝合有利于剖宫产切口愈合,瘢痕憩室程度可得到改善。Objective To evaluate the influence of different cesarean section suture on the formation of uterine incision scar diverticulum. Methods 365 cases of elective cesarean section puerpera in Fifth People ＇s Hospital of Dongguan City from March 2014 to September 2015 were selected and divided into research group and control group. Research group were treated with double continuous suture, control group were treated with single - layer continuous suture. Diverticulum muscle thickness, diverticulum formation cases, diverticulum volume, intraoperative blood loss, operation time, anal exhaust time, postoperative lochia duration and hospital stay were compared between the two groups. Results There was no significant difference between the two groups in the amount of blood loss, operation time, anal exhaust time, duration of postoperative lochia and hospital stay （P 〉 0. 05 ）. Diverticulum formation rate and diverticulum volumevs of the research group [ 3.83 %, （0. 36± 0. 12）mL ] were lower than that of the control group [ 9.34% , （0. 49 ±0. 09）mL] ,and diverticulum muscle thickness[ （7. 63 ± 1.92） mm] were higher than the control group[ （5.48 ± 2. 51 ） mm]. The differences were statistically significant （ P 〈 0.05 ）. Conclusion Double continuous suture is conducive to the healing of cesarean section incision, scar diverticulum degree could be improved.
