不同剂量氨溴索治疗中老年社区获得性肺炎疗效观察

摘　　要：目的:探讨不同剂量氨溴索治疗中老年社区获得性肺炎的疗效。方法:收集老年肺炎患者分为:观察组(盐酸氨溴索15mg/kg·d)和对照组(盐酸氨溴索9 mg/kg·d)。对比2组治疗前后血气分析结果、咳痰好转时间及住院时间。结果:(1)2组治疗前血气分析比较差异无统计学意义(P>0.05)。2组治疗后呼吸道咳嗽症状评分及血气分析比较差异有统计学意义(P<0.05)。(2)观察组和对照组咳痰好转时间及住院时间分别为(4.6±1.2)d,(6.8±2.4)d,(5.1±0.8)d,(7.9±2.7)d,2组比较差异有统计学意义(P<0.05)。(3)观察组和对照组治疗前CRP及D-二聚体分别为(68.5±18.5)mg/L,(1342.6±485.7)ng/m L,2组比较差异无统计学意义(P>0.05);观察组和对照组治疗后CRP及D-二聚体分别为(152.2±49.6)mg/L,(2975.3±756.2)ng/m L,差异有统计学意义(P<0.05)。结论:治疗中老年社区获得性肺炎15 mg/kg·d盐酸氨溴索效果优于9 mg/kg·d盐酸氨溴索。Objective： To investigate the value of different doses of ambroxol hydrochloride in treatment of community acquired pneumonia in the elderly. Methods： Senile patients with pneumonia in our hospital were selected and divided into a study group（ ambroxol hydrochloride treatment,15 mg / kg · d） and a control group（ ambroxol hydrochloride treatment,9 mg / kg · d）. The two groups were compared before and after the treatment in terms of the results of blood gas analysis,sputum improvement time and hospitalization. Results：（ 1） There were no differences in blood gas analysis between the two groups before the treatment（ P〈0. 05）. There were statistical differences in the two groups after the treatment（ P〉0. 05） in respiratory cough symptom scores and blood gas analysis.（ 2）The sputum improvement time and hospitalization of the study group and the control group were respectively（ 4. 6 ± 1. 2） d,6. 8 ± 2. 4）d,（ 5. 1 ± 0. 8） d,（ 7. 9 ± 2. 7） d,which had significant difference（ P〈0. 05）.（ 3） CRP and D-dimer of the study group and the control group showed no difference before treatment; after the treatment they were statistically significant（ P〈0. 05）. Conclusion： Therapy of ambroxol hydrochloride 15 mg / kg·d is better than 9 mg / kg·d in treatment of community acquired pneumonia in the elderly.
