DLACF切开复位内固定手术疗效的相关影响因素分析

摘　　要：目的研究移位关节内跟骨骨折(DLACF)切开复位内固定手术疗效的相关影响因素分析。方法选取进行切开复位内固定手术的33例移位关节内跟骨骨折患者(DLACF)作为研究对象,根据手术疗效的可能影响因素,对患者的年龄、体重、术前B角、术后G角、术前时间以及Sanders分型等情况进行检测与记录,将结果进行比较分析。结果 33例患者共37足的术后疗效,其中优18例,良12例,可5例,差2例,在各因素中,体重、术前B角、术后G角、术前时间等对手术的疗效有一定的影响,差异有统计学意义(P<0.05),而年龄对术后等等疗效没有影响,差异无统计学意义(P>0.05)。Ⅱ型Sanders分型的患者总优良率为94.1%,Ⅲ型患者的总优良率为73.3%,Ⅳ型患者的总优良率为60.0%,差异有统计学意义(P<0.05)。将年龄、体重、术前B角、术后G角、术前时间、Sanders分型等因素进行多因素Logistic回归分析,结果显示,体重、术前B角、术后G角、术前时间、Sanders分型等是患者术后疗效的评分的影响因素。结论对于DLACF切开复位内固定手术,其术后疗效的影响因素主要有体重、术前B角、术后G角、术前时间以及Sanders分型等,在手术过程中应尽量避免不良因素,提高患者的手术疗效。Objective To investigate the related influencing factors on the therapeutic effects of open reduction and internal fixation （ORIF） in treatment of displaced intra-artieular ealeaneal fracture （DLACF）. Methods Thirty-three patients with DLACF who were treated by open reduction and internal fixation in our hospital were enrolled in the study. According to possible influencing factors on surgical therapeutic effects, patient＇ s age, weight, preoperative angle B, postoperative angle G, preoperative time and Sanders typing were detected and recorded, and the examination results were statistically analyzed. Results The therapeuitic effects of ORIF surgery, among 33 patients with DLACF, 18 cases were excellent,12 cases were good, 5 cases were all right, 2 cases were poor. The patient＇s weight, preoperative angle B, postoperative angle G, preoperative time had significant effects on surgical therapeutic effects （ P 〈 0. 05 ） , however, patient＇ s age had no obvious effects on surgical therapeutic effects. The total excellent rate in patients with type Ⅱ （ Sanders classification） was 94.1% ,which was 73.3% in patients with type Ⅲ and 60.03% in patients with type Ⅳ,there were significant differences among them （ P 〈 0.05 ）. The multivariate Logistic regression analysis showed that body weight, preoperative angle B, postoperative angle G, preoperative time, Sanders typing were influencing factors on postoperative therapeutic effects scoring. Conclusion The influencing factors on the therapeutic effects of open reduction and internal fixation in treatment of DLACF are mainly patient＇ s body weight, preoperative angle B, postoperative angle G, preoperative time and Sanders typing,therefor, these factors should be avoided during surgery to improve surgical efficiency.
