摘 要:目的:观察针刺"夹脊"穴预处理对大鼠急性心肌缺血再灌注损伤(myocardial ischemiareperfusion injury,MIRI)的保护作用,并初步探讨其作用机制。方法:将50只Wistar大鼠随机分为对照组、模型组、夹脊组、内关组、曲池组,每组10只。夹脊组、内关组、曲池组于造模前分别针刺T4~T5"夹脊""内关""曲池"穴进行电针预处理7d;对照组和模型组常规饲养,不进行针刺。针刺结束后,除对照组外,余组均采用结扎左冠状动脉前降支(LAD)的方法复制MIRI模型。监测大鼠心电图,分析ST段位移情况;采用HE染色法观察各组大鼠心脏组织的形态,并运用透射电镜观察心肌细胞超微结构;采用WST-1法测定血清超氧化物歧化酶(SOD)活性,TBA法测血清丙二醛(MDA)含量;实时荧光定量PCR法测定缺血区心肌组织Nrf 2及下游HO-1基因的表达情况。结果:与对照组比较,余组LAD结扎后,心电图ST段均明显抬高,再灌注后回落(均P<0.05);缺血30min时,夹脊组、内关组ST段抬高幅度低于模型组(均P<0.05)。与模型组相比,夹脊组、内关组SOD活力明显上升(均P<0.05),MDA含量明显下降(均P<0.05),且"夹脊"穴效果优于"内关"穴(均P<0.05)。病理方面,"夹脊"穴与"内关"穴均能良好改善心肌组织形态及细胞超微结构,且夹脊组效果更显著,曲池组改善不明显。与对照组比较,模型组心肌组织中Nrf 2及HO-1基因的表达下调(均P<0.05),夹脊组、内关组较模型组明显上调(P<0.05,P<0.01),且夹脊组上调倍数更高。结论:"夹脊"穴预处理对大鼠心肌缺血再灌注损伤有保护作用,其作用机制可能与上调Nrf 2-ARE通路表达、激活内源性抗氧化通路、提高氧自由基清除能力、减少脂质过氧化物损伤有关。Objective To observe the protective effect of acupuncture preconditioning at"Jiaji"(EX-B 2)on acute myocardial ischemia-reperfusion injury(MIRI)in the rats and explore the mechanism.Methods Fifty Wistar rats were randomly divided into a control group,a model group,a Jiaji group,a Neiguan group and a Quchi group,10 rats in each one.In the Jiaji group,the Neiguan group and the Quchi group,electroacupuncture was given for preconditioning at"Jiaji"T4-T5(EX-B 2),"Neiguan"(PC 6)and"Quchi"(LI 11)for 7days before modeling.In the control group and the model group,the regular feeding was given,without any acupuncture.At the end of acupuncture,except the control group,ligating the left anterior descending coronary artery(LAD)was adopted to duplicate MIRI models in the rest groups.Electrocardio-gram(ECG)was monitored and ST-segment shift was analyzed.HE staining method was adopted to observe the morphology of cardiac tissue in the rats of the groups.The transmission electron microscope was used to observe myocardial cell ultrastructure.WST-1method was used to determine the activity of serum superoxide dismutase(SOD),TBA method was used to determine the content of serum malondialehyde(MDA)and the real-time fluorescent quantitative PCR method to determine theexpressions of Nrf 2in ischemic myocardial tissue and downstream HO-1gene.Results Compared with the control group,after LAD ligation,ST-segment was elevated and depressed in ECG apparently after reperfusion in the rest groups(all P〈0.05).The ST-segment elevation in the Jiaji group and the Neiguan group was less than that in the model group(both P〈0.05).Compared with the model group,SOD activity was increased apparently in the Jiaji group and the Neiguan group(both P〈0.05),and MDA content was reduced apparently(both P〈0.05).The effects in the Jiaji group were better than those in the Neiguan group(both P〈0.05).Pathologically,"Jiajia"(EX-B 2)and"Neiguan"(PC 6)all improved the mo
