摘 要:目的探讨体质量指数(BMI)与臂踝动脉脉搏波传导速度(baPWV)的关系。方法以参加2010-2011(2010)、2012-2013(2012)、2014-2015(2014)年健康体检且参加baPWV检测的开滦集团职工22 681人为研究对象,排除了身高、体质量资料缺失者3009人,最终纳入统计分析的观察对象共计19 672人。按BMI将观察对象分成3组:正常组(BMI<24kg/m^2)、超重组(BMI 24~<28kg/m2)和肥胖组(BMI≥28kg/m^2),采用多因素Logistic回归模型分析不同BMI分组及BMI每增加一个标准差对baPWV≥14 m/s的影响。结果不同BMI分组间baPWV≥14m/s的检出率分别为:48.4%、60.8%、64.0%;多因素Logistic回归分析显示,在校正了年龄、性别、是否吸烟、饮酒、体育锻炼后,与正常组相比,baPWV≥14m/s的发生风险随BMI增加而呈上升趋势,然而进一步校正了平均动脉压(MAP)、心率、是否高血压、糖尿病、血脂异常、是否使用降压药、降糖药、调脂药等混杂因素后,超重和肥胖组发生baPWV≥14m/s的OR值(95%CI)分别为0.96(0.88~1.04)、0.85(0.76~0.96);另外,BMI每增加一个标准差发生baPWV≥14m/s的OR值(95%CI)为0.94(0.90~0.98)。结论在一般人群中BMI不是与baPWV关联良好的指标。Objective To evaluate the association between body mass index(BMI)and brachial-ankle pulse wave velocity (baPWV). Methods A total of 22 681 Kailuan Group workers who participated in 2010--2011 (2010), 2012--2013 (2012) , 2014--2015 (2014) health examination and received baPWV measurement were included as the research subject. Excluding 3009 height and weight data missing, there were 19 672 individuals finally included in the statistical analysis. According to the body mass index (BMI), subjects were divided into three groups: normal weight group (BMI〈24 kg/m^2), overweight group (BMI 24-〈28 kg/m^2) and obese group (BMI≥28 kg/m^2). The multivariate Logistic regression model was used to analyze the effect of different groups of BMI and an increase in a standard deviation of BMI on the baPWV≥14 m/s. Results The prevalences of baPWV≥14 m/s in different BMI groups were 48.4%, 60.8% and 64.0% respectively. After adjusting for confounding factors such as age, gender, smoking, drinking and physical exercise, the risk of baPWV≥14 m/s increased with the increase of BMI. However, when going on adjusting for mean arterial pressure (MAP), heart rate, hypertension, diabetes mellitus, dyslipidemia and other confounding factors, the multivariate Logistic regression analysis showed that the OR (95% CI) were 0. 96(0. 88-1. 04) and 0.85 (0.76 0.96) respectively for baPWV≥ 14 m/s in overweight and obese group compared to normal BMI group; and as BMI increased a standard deviation, the OR (95% CI) was 0.94 (0.90-- 0.98). Conclusion In the general population, BMI is not a good indicator for evaluating baPWV.
