二维斑点追踪技术定量评价重度子痫前期患者左室收缩功能

摘　　要：目的应用左室纵向应变评估射血分数正常的重度子痫前期患者左室收缩功能。方法选取重度子痫前期患者52例作为病例组,40例年龄、孕周匹配的健康孕妇作为正常组,所有入选者LVEF≥50%,产前及产后均采集二维斑点追踪技术所需相关切面图像,计算各切面左室整体纵向应变(GLS)及左室整体纵向平均应变(GLS-Avg)。结果病例组产前2C-GLS、4C-GLS、LAX-GLS、GLS-Avg均低于正常组产前(P<0.05);病例组产后2C-GLS、4CGLS、GLS-Avg均低于正常组产后(P<0.05);病例组产前2C-GLS、4C-GLS均低于病例组产后(P<0.05);正常组产前2CGLS、4C-GLS、LAX-GLS、GLS-Avg均低于正常组产后(P<0.05)。结论射血分数正常的重度子痫前期患者存在早期左室收缩功能的损害;通过二维斑点追踪技术中左室纵向应变的减低,能够识别早期左室收缩功能损害。Objective To evaluate the left ventricular systolic function in patients of severe preeclampsia with normal ejection fraction using left ventricular longitudinal strain. Methods 52 patients of severe preeclampsia were se- lected as the case group,40 patients of healthy pregnant women gestational weeks-age-matched were used as the con- trol group. Ejection fraction of all the selected subjects were greater than or equal to 50~. The relevant section of the images were acquired and analysed offline using 2D-STI, then we calculated the left ventricular GLS and GLS-Avg. All subjects repeated the above operation after delivery. Results Compared with the control group before delivery, 2~ GLS,4C-GLS,LAX-GLS and GLS-Avg decreased in case group （P〈0.05） ;Compared with the control group after de- livery, 2C-GLS,4C-GLS,GLS-Avg decreased in case group（P〈0.05）;Compared with the case group, 2C-GLS and 4C-GLS increased in case group after delivery（P〈0.05） ;Compared with the control group, 2C-GLS, 4C-GLS, LAX-GLS and GLS-Avg increased in control group after delivery（P〈0.05）. Conclusions Patients of Severe preeclampsia with normal ejection fraction exist damage of left ventricular systolic function ; We can identify it using 2D-STI.
