摘 要:目的探讨超声及超声造影(CEUS)对宫颈癌化疗疗效评估的临床价值。方法选择在我院接受新辅助化疗的宫颈癌患者31例,在新辅助化疗前和化疗后分别进行二维超声及超声造影检查,观察化疗前后肿瘤大小形态变化,根据RECIST评价体系分析宫颈肿瘤缓解情况,并与术后病理结果进行相关性分析,利用时间-强度曲线定量分析多组造影参数在新辅助化疗前后的改变。结果超声测量显示治疗后,肿瘤完全缓解2例,部分缓解15例,肿瘤缓解不明显14例,通过与病理检查结果相比,肿瘤化疗疗效与肿瘤细胞对肌层浸润深度具有明显相关性(P<0.05);宫颈癌患者经新辅助化疗后PI、AUC显著低于治疗前,而MTT、TP较治疗前增高,差异有统计学意义(P<0.05)。结论二维超声及超声造影检查能相对准确的检测宫颈癌新辅助化疗前后,肿瘤大小及肿瘤血流灌注改变,对宫颈癌化疗疗效评估有一定参考价值。Objective The purpose of this study was to investigate 2D ultrasound and contrast-enhanced ultra- sound assessment the effect of chemotherapy on cervical carcinoma. Methods 2D ultrasound and Contrast-enhanced ultrasound was performed before and after neoadjuvant chemotherapy in 31 patients diagnosis of cervical carcinoma. Compare of the size and characterize of cervical tumors, all the results were scheduled to compare with pathological di- agnosis. Analyse the multi-parameters of contrast-enhanced ultrasound by time-intensity curve before and after the chemotherapy. Results There were 17 cases had response to neoadjuvant chemotherapy and 14 cases do not have re- sponse to neoadjuvant chemotherapy. The size of cervical cancer before and after neoadjuvant chemotherapy have rela- tive with pathological examination. The level of PI, AUC after chemotherapy were significantly lower than those before treatment in cervical cancer and the level of MTT,TP after chemotherapy were significantly longer than those be- fore treatment. Conclusions 2D ultrasound and Contrast-enhanced ultrasound can measure the lesion of the cervical cancer before and after the neoadjuvant chemotherapy. TIC parameters can reflect the perfusion mode changes of cervical cancer effectively, which may provide available evaluation of chemotherapy.
