摘　　要：目的比较分析河南省开封市儿童医院就诊患儿和健康儿童麻疹抗体水平,为制定麻疹院内传播防控策略提供依据。方法采用酶联免疫吸附试验(ELISA)对开封市儿童医院0-4岁就诊患儿和健康儿童进行麻疹IgG抗体水平血清学检测,并进行流行病学分析。结果 812名0-4岁患儿麻疹抗体阳性率为48.28%,抗体几何平均浓度(GMC)为103.02 IU/L;其中517名0-7月龄患儿抗体阳性率和GMC分别为42.75%、86.99 IU/L;295名8月龄-4岁患儿抗体阳性率和GMC分别为57.97%、138.57 IU/L(χ~2=17.42,P<0.001;t=2.54,P=0.01)。165名8月龄-4岁健康儿童抗体阳性率和GMC分别为96.36%、920.71 IU/L,高于同年龄患儿(χ~2=76.95,P<0.001;t=8.62,P<0.001)。8月龄-4岁患儿含麻疹成分疫苗(MCV)接种率为78.64%,低于同年龄健康儿童的99.39%;接种过MCV的患儿或健康儿童抗体阳性率和GMC均高于未种患儿或健康儿童。结论开封市儿童医院就诊患儿麻疹抗体水平低,需研究综合性防控措施,提高患儿麻疹免疫水平,防止患儿的麻疹院内感染。Objective To compare measles antibody levels among patients seeing doctors in Kaifeng Children＇s Hospital with levels among healthy children,providing evidence for developing a strategy to prevent nosocomial infection.Methods Enzyme linked immunosorbent assay（ELISA） was used to detect measles Ig G antibody levels among 0-4 year old patients and among healthy children.Data were analyzed descriptively.Results The positivity rate and geometric mean concentration（GMC） of measles antibody were 48.28% and 103.02 IU/L,respectively,among 812 patients aged 0-4 years;were42.75% and 86.99 IU/L among 517 patients aged 0-7 months,and were 57.97% and 138.57 IU/L among 295 patients aged 8 months to 4 years（χ^2=17.42,P〈0.001;t=2.54,P=0.01）.The positivity rate and GMC among healthy children aged 8 months to 4 years were 96.36% and 920.71 IU/L,respectively,higher than among same-age patients（χ^2=76.95,P〈0.001;t=8.62,P〈0.001）.Coverage with measles containing vaccine（MCV） among patients aged 8 months to 4 years was 78.64%,lower than the 99.39% among healthy children in the same age.The positivity rate and GMC were both higher among healthy children or patients who received MCV than among those who did not receive MCV.Conclusions Measles antibody levels were low among patients in Kaifeng Children＇s Hospital.Comprehensive preventive measures should be taken to increase their immunity to prevent nosocomial measles infection.
