瑞芬太尼复合丙泊酚恒速输注在全凭静脉麻醉中的应用研究

摘　　要：目的探讨和分析瑞芬太尼复合丙泊酚恒速输注在全凭静脉麻醉中的应用效果。方法选取2013年11月—2016年11月在本院接受临床治疗的200例ASAⅠ~Ⅱ级全麻患者作为研究对象,观察患者在诱导前(T_1)、插管后即刻(T_2)、插管后5 min(T_3)、手术进行1 h(T_4)、拔管即刻(T_5)、拔管后3 min(T_6)的HR、SBP、DBP、PETCO_2,及停药后自主呼吸恢复时间、睁眼时间以及苏醒后躁动的发生频率和停用瑞芬太尼前后的MAP变化。结果患者不同时间点的HR、SBP、DBP对比,差异均不具有统计学意义(P>0.05)。患者麻醉维持时间、咽喉反射时间、拔管时间、正确回答问题时间分别为(89.0±3.1)min、(4.2±1.9)min、(5.3±3.0)min、(8.0±1.7)min。结论瑞芬太尼复合丙泊酚恒速输注在患者的全凭静脉麻醉中的应用效果较好,机体生命体征较为稳定,有利于手术顺利进行。Objective To investigate and analyze application of the effecton remifentanil compound with propofol infusion of constant speed in total intravenous anesthesia. Methods From November 2013 to November 2016 in our hospital, 200 cases of patients with ASA Ⅰ ~ Ⅱ level general anesthesia were selected as the research object, to observe the patients before the induction（T1）, immediately after intubation（T2）, 5 min after intubation（T3）, 1 h surgery（T4）, tube drawing immediate（T5）, 3 min after extubation（T6） HR, SBP, DBP, PETCO2, and spontaneous breathing recovery time after the drug was stopped, open time and agitation after waking frequency and disable the MAP changes before and after the fentanyl. Results Compared with HR, SBP and DBP at different time points, the difference was not statistically significant（P〉0.05）. The duration of anesthesia maintenance, throat reflex time, extubation time, correct answer time were（89.0±3.1） min,（4.2±1.9） min,（5.3±3.0） min,（8.0±1.7） min. Conclusion The application of remifentanil combined with propofol in patients undergoing total intravenous anesthesia is effective, the vital signs were stable, which was conducive to the smooth operation.
