输卵管阻塞性不孕经介入治疗后再通效果的影响因素

摘　　要：目的:探讨输卵管阻塞性不孕经介入治疗后再通效果的影响因素。方法:诊断为输卵管近端阻塞性不孕症并且符合纳入标准的260例患者作为研究对象,采用输卵管介入再通术(fallopian tube interventional reperfusion,FTR),并经微导管直接造影和注入药物,再通未成功的患者作为观察组,再通成功的患者作为对照组。使用同一问卷对两组患者的年龄、职业、不孕时间、盆腔炎、宫内节育器放置、人流刮宫史等进行调查。应用单因素和多因素分析方法对有关因素进行分析。结果:单因素分析显示再通效果与盆腔炎、盆腹腔手术史、宫内节育器放置、输卵管造影次数有相关性(P<0.05)。非条件Logistic因素回归分析发现,盆腹腔手术史是影响输卵管再通效果的独立危险因素(OR=2.142,P=0.003)。结论:盆腹腔手术史是输卵管再通效果的独立危险因素。Objective：To study the influencing factors on the effect of interventional treatment of fallopian tube obstructive infertility. Meth- ods： 260 patients, diagnosed as proximal tubal obstructive infertility and accorded with the inclusion criteria as the research object, underwent the fallopian tube recanalization （FTR） operation and was directed angiography and drug injection by micro catheter. Cases of successful recanalization were assigned in the observation group and cases of unsuccessful recanalization were in the control group, with age, occupation, time of infertility, pelvic inflammatory disease, intrauterine device, and curettage abortion history of the two groups investigated using the same questionnaire. Single factor and multi factor analysis were used to analyze the related factors. Results： Single factor analysis showed that there was correlation between the effect of reeanalization and pelvic inflammatory disease, pelvic cavity surgery history, intrauterine device placement, and the number of tubal contrast （ P 〈 0.05 ）. Non - conditional Logistic regression analysis showed that the history of pelvic and abdominal surgery was an independent risk factor for the effect of fallopian tube reeanalization（ OR = 2. 142, P = 0. 003 ）. Conclusion ： The history of pelvic and abdominal surgery is an independent risk factor for tubal reeanalization.
