摘 要:目的探讨25(OH)D与2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)的关系。方法采集T2DM合并NAFLD患者(DNF组,60例),T2DM未合并NAFLD组患者(DM组,60例)及体检健康者(DC组,60例)的血清25(OH)D浓度及生化、C肽和24h尿微量白蛋白(24UAMLB),同时测定血糖参数进行比较。结果 DNF组25(OH)D与Ur(r_s=0.175,P<0.05)呈正相关,与24UAMLB(r_s=-0.145,P<0.05)、AST/ALT(r_s=-0.127,P<0.05)均呈负相关;AST/ALT为25(OH)D的危险因素,尿素为25(OH)D的保护因素。结论合并NAFLD的T2DM患者血清维生素D水平更低;监测血清25(OH)D的水平有助于评估2型糖尿病患者并发非酒精性脂肪性肝病的风险性。Objective To explore the relationship between 25(OH) D and type 2 diabetes mellitus(T2DM)combined with non alcoholic fatty liver disease(NAFLD).Methods The clinical and biochemical indexes,including serum 25(OH)D,from 60 patients with T2 DM complicated with NAFLD(group DNF),60 patients with T2 DM but without NAFLD(group DM),and 60 healthy people(group NC) were collected and analyzed.Results In group DNF,25(OH) D was positively correlated with Alb and Ur(P〈0.05),and negatively with24 h urinary microalbumin(24UAMLB),aspartate aminotransferase/alanine aminotransferase(AST/ALT) and apolipoprotein A1(ApoA1)(P〈0.05);Uric acid(Ur) was the independent protective factor of 25(OH) D,while AST/ALT and 24 UAMLB were the independent risk factors of 25(OH) D.Conclusion The serum levels of vitamin D were lower in the patients with T2 DM combined with NAFLD.Monitoring the level of serum 25(OH) D is beneficial to assess the risk of non alcoholic fatty liver disease in patients with type 2 diabetes mellitus.
